A Controversial New Cosmetic Filler Made From Cadavers Is Reshaping Butt Lifts and Breast Implants

As people age, their skin naturally changes — wrinkles deepen, breasts lose firmness and facial volume fades. While many accept these changes, millions around the world turn to cosmetic procedures to preserve a youthful appearance. From Botox to dermal fillers and surgical implants, aesthetic medicine has become a global industry, with demand continuing to surge.

According to a 2023 report by the International Society of Aesthetic Plastic Surgery (ISAPS), more than 19 million non-surgical cosmetic procedures were performed worldwide. Yet even as these treatments become routine, many patients are unaware of a controversial new material being used in some of today’s most popular body-contouring procedures.

That material is fat taken from deceased donors.

A new cosmetic product known as AlloClae has begun circulating in clinics across the United States, offering surgeons a novel way to add volume to the body without harvesting fat from a patient’s own tissue. The injectable filler is made from purified human fat collected from cadaver donors and is being used in Brazilian butt lifts (BBLs), breast enhancements and hip-dip corrections.

The growing demand for donor-derived fat is being driven partly by the popularity of weight-loss drugs, which can leave patients with less body fat available for traditional fat-transfer procedures, and partly by the search for minimally invasive alternatives to surgery. While many plastic surgeons have welcomed the innovation, the practice has triggered medical, ethical and even spiritual debates among patients and critics.

What is AlloClae?

Launched in the U.S. market in 2024, AlloClae is a sterile injectable filler made from human fat harvested from deceased donors. Before it is used in patients, the fat undergoes a multi-step purification process designed to remove DNA, cellular debris and anything that could provoke an immune response.

The final product is intended to integrate into the recipient’s body and stimulate collagen growth, restoring or adding volume in areas where fat naturally accumulates, such as the breasts, buttocks and hips.

Why doctors are using it

Traditionally, cosmetic fat transfers require liposuction to remove fat from a patient’s own body before it can be reinjected elsewhere. But not every patient has enough excess fat to make that option viable.

“This is great for somebody who doesn’t want to use their own fat or doesn’t have enough of their own fat to do a Brazilian butt lift or buttock augmentation,” New York plastic surgeon Dr. Stephen T. Greenberg said in a YouTube video demonstrating the procedure.

Dr. Sachin M. Shridharani told The New York Post that only a small fraction of board-certified plastic surgeons currently offer the procedure, but demand is rapidly growing. “I’d say less than probably 5% of board-certified plastic surgeons have it,” he said, adding that he has performed more than 50 procedures in the past year and has at times run out of product.

Regulatory status and concerns

AlloClae is classified in the U.S. as a human cell and tissue product, which means it does not require full Food and Drug Administration approval in the same way drugs and medical devices do. Doctors say it is biocompatible and designed to work safely within the body.

Still, the idea of injecting fat derived from deceased donors into living patients has raised questions that go beyond medicine. For some, the ethical and emotional implications are unsettling, even if the science supports its safety.

As the popularity of AlloClae grows, patients considering cosmetic enhancements are being faced with a new and deeply personal decision: whether achieving a desired body shape is worth using tissue that once belonged to someone else.
