A new study published in the British Journal of Sports Medicine reveals that older women who achieve at least 4,000 steps per day for 1 to 2 days per week have a lower risk of all-cause mortality and cardiovascular disease. The research followed 13,547 women aged 45 and older without cardiovascular disease or cancer, tracking their daily steps over a seven-day period and monitoring outcomes for about 11 years.

The shift to sedentary lifestyles with industrialization has reduced daily physical activity from roughly 15,000–20,000 to about 5,000 steps among older adults. Regular physical activity is associated with better health and longevity, but guidelines continue to refine what constitutes meaningful movement for older populations. Study design
Participants wore a step-count device for seven days and were followed for approximately 11 years. The analysis evaluated four daily step thresholds: 4,000, 5,000, 6,000, and 7,000 steps per day. Participants were grouped by how many days per week they met these thresholds: 0 days, 1–2 days, or 3 or more days. The study population was predominantly older, white, U.S. women with higher socioeconomic status, which may limit generalizability.

While higher daily steps correlate with better outcomes, the observational design means associations do not prove causation. Additional research suggests that faster walking pace and longer distance may offer extra protection, particularly in higher-risk populations. Other studies also highlight mental health benefits, with each 1,000-step increase linked to a roughly 9% reduction in depression risk. Urban design promoting walkability can sustainably raise daily step counts in populations.
Clinical and public health relevance
The study supports incorporating step metrics into physical activity guidelines and clinical practice, especially for older women. It suggests that encouraging even small, achievable increases in daily steps can contribute to meaningful health gains in real-world settings.
