PCOS Management Crucial for Preventing Gynaecological Cancer Risks

Women with metabolic syndrome (MetS) are at a significantly higher risk of developing several gynaecological cancers, according to a systematic review published in the Indian Journal of Medical Research (IJMR). The collaborative study was carried out by experts from the ICMR–National Institute of Reproductive and Child Health (NIRRCH), Mumbai, M.S. Ramaiah University of Applied Sciences, Bengaluru, and Amrita Institute of Medical Sciences (AIMS), Kochi. It assessed global evidence linking MetS with cancers of the ovary, endometrium, cervix, vagina, and vulva.

Defining Metabolic Syndrome and Its Growing Prevalence

Metabolic syndrome is defined as a complex cluster of metabolic disorders including obesity, hypertension, hyperglycaemia, elevated triglycerides, and low high-density lipoprotein (HDL) cholesterol. Its prevalence increases sharply with age—from about 13% among young adults (18–29 years) to nearly 50% by age 50—and it is more common in women (35%) than men (26%).

This widespread metabolic condition has become an important global public health concern, particularly because of its close association with cardiovascular diseases, diabetes, and now, as evidence suggests, several forms of gynaecological cancer.

Study Findings Across Research Designs

The review analyzed data from various study types including case-control, cross-sectional, and cohort studies. The results consistently showed that women with metabolic syndrome had a markedly higher likelihood of developing gynaecological cancers.

Case-Control Studies

Case-control data revealed nearly double the odds of endometrial cancer among women with MetS (odds ratio, OR 1.99). The risk for ovarian cancer was even greater, with an OR of 3.44, indicating a more than threefold increase in likelihood. Cervical cancer also showed elevated odds (OR 1.9), while smaller but significant associations were noted for both vaginal and vulvar cancers.

Cross-Sectional and Cohort Studies

Cross-sectional analyses indicated that the odds of endometrial cancer were more than twice as high (OR 2.64) among women with MetS. Cohort studies—considered more robust due to their long-term tracking—showed increased risks across several cancer types: a 45% higher risk for endometrial cancer (hazard ratio, HR 1.45), 26% for cervical cancer (HR 1.26), 49% for vulvar cancer (HR 1.49), and 54% for vaginal cancer (HR 1.54).

India-Specific Gaps in Research

Despite India’s growing burden of metabolic syndrome and related non-communicable diseases, the researchers found no Indian studies that could be included in the review. Dr. Susan Idicula-Thomas, the corresponding author and scientist at ICMR–NIRRCH, noted, “It was surprising to see a complete lack of India-specific data. This highlights a major gap in understanding comorbidity risks among Indian women.” She emphasized that without population-specific evidence, it is difficult to develop tailored prevention and management strategies.

Challenges in Data Comparison

First author Indra Kundu, a PhD scholar at ICMR–NIRRCH, explained that variations in study designs and demographic characteristics posed challenges in directly comparing the magnitude of cancer risks across populations. However, the consistency in findings across regions strongly supports a biological link between metabolic disorders and gynaecological cancer development.

Public Health Implications and Preventive Strategies

Highlighting the public health impact, Denny John from M.S. Ramaiah University of Applied Sciences stated, “The findings reinforce the need to integrate women with existing metabolic disorders, including diabetes, into preventive health programmes. Such integration is critical for both primary prevention and early detection of gynaecological cancers.”

Recommendations for Clinical and Public Health Practice

Based on the review, the authors advocate for comprehensive PCOS management and metabolic syndrome control as central components of cancer prevention frameworks. The study recommends proactive monitoring, lifestyle interventions, and early screening for women diagnosed with metabolic conditions. These measures are essential not only for managing metabolic syndrome but also for reducing the long-term risk of cancer development.

Conclusion

The study underscores the urgent need for India-specific research on metabolic syndrome and cancer comorbidities. Furthermore, it calls for coordinated healthcare strategies that combine metabolic management, reproductive health, and cancer screening. Effective PCOS management and metabolic regulation could thus play a pivotal role in reducing the rising incidence of gynaecological cancers among women worldwide.
