“Ozempic face” has become a widely discussed term among people using the diabetes drug Ozempic for weight loss, but it’s more than just a buzzword. Bariatric surgeon Andrea Bedrosian, MD, of Northwell Health, explains the causes, potential solutions, and what patients should know.

Originally developed to treat type 2 diabetes, Ozempic has gained attention for its effectiveness in promoting rapid weight loss. However, this speed can sometimes result in noticeable facial changes, described as “Ozempic face.”

What is Ozempic Face?

“Ozempic face” is not a medical diagnosis but refers to the gaunt or hollowed appearance some people experience during rapid weight loss. According to Dr. Bedrosian, “Ozempic will help you lose a lot of weight in a relatively short period. A lot of people will see that initial weight loss in their face first.”

The loss of facial fat can make cheeks and temples appear sunken, highlight bone structure, and contribute to sagging skin and accentuated wrinkles. Dr. Bedrosian emphasizes that these changes result from rapid weight loss itself, not directly from Ozempic. Similar effects can occur with bariatric surgery or restrictive diets.

“It’s a pretty common thing,” she notes. “You’re just hearing about it more because so many more people are on this medication. Ultimately, it’s a sign of anyone losing a significant amount of weight quickly.”

Beyond the Face: “Ozempic Butt”

Rapid weight loss can also affect other areas with higher fat content, such as the buttocks, belly, and thighs, leading to sagging skin. While less commonly reported than facial changes, these effects are part of the same process.

How Ozempic Works

Ozempic belongs to a class of drugs called GLP-1 receptor agonists, which mimic a naturally occurring hormone that regulates blood sugar and appetite. By increasing satiety and reducing hunger, the drug helps people eat fewer calories and lose weight. Other similar medications include Wegovy, Mounjaro, and Zepbound.

Dr. Bedrosian notes that while bariatric surgery remains the most effective option for significant weight loss, medications like Ozempic provide an alternative for those who cannot or do not want surgery.

Managing Ozempic Face

Although rapid weight loss has clear health benefits, the cosmetic changes it brings are valid concerns. Dr. Bedrosian recommends several strategies:

Gradual weight loss: Slower progress allows skin time to adapt, minimizing sagging. Discuss dosage adjustments and goals with a doctor or registered dietitian.

Dermal fillers: Injectable treatments can restore lost facial volume.

Healthy lifestyle: Balanced nutrition, regular exercise, and proper hydration support skin elasticity.

For some, surgical options may be considered, but experts advise caution. “The first step should be to address the rapid weight loss that caused the facial changes,” Dr. Bedrosian says. Often, skin may naturally regain some elasticity over time, depending on age, prior weight, and how long a person lived with overweight or obesity.

She also warns against using these medications purely for cosmetic purposes, highlighting the potential health risks for individuals without medical need. Both Ozempic and Wegovy have experienced ongoing FDA drug shortages due to high demand.

Other Considerations and Benefits

GLP-1 receptor agonists can also cause gastrointestinal issues, pancreatitis, thyroid tumors, gallstones, hair loss, and mental health effects. Thorough patient education and screening are essential.

Beyond weight loss, Ozempic offers additional health benefits, including improved diabetes control, kidney function, and cardiovascular health. Research suggests some effects, like kidney protection, may occur independently of weight loss, though further studies are needed in diverse populations.

Dr. Bedrosian stresses that weight management is complex and that medications should be used responsibly, in combination with lifestyle changes and professional guidance.
