PARP Inhibitors Improve Disease Control in Advanced Ovarian Cancer

A recent meta-analysis examined the role of poly (ADP-ribose) polymerase (PARP) inhibitors as maintenance therapy in patients with newly diagnosed advanced ovarian cancer. Researchers pooled data from seven randomized controlled trials, comprising 11 studies and 3,220 patients. Both pairwise and Bayesian network meta-analyses were used to compare outcomes, with progression-free survival (PFS) and adverse events as the primary endpoints. The study provides a comprehensive assessment of efficacy and safety across the PARP inhibitor class in this setting.

Progression-Free Survival Benefits Across Genomic Subgroups

The analysis found that maintenance therapy with PARP inhibitors significantly improved PFS compared with non-PARP regimens. Benefits were observed across all genomic subgroups, including patients with BRCA mutations, BRCA wild-type tumors, homologous recombination deficiency (HRD), and homologous recombination proficiency (HRP). However, the meta-analysis did not show an overall survival advantage for the overall cohort or the HRD subgroup. Within the network comparison, senaparib demonstrated superior PFS efficacy compared with veliparib and niraparib, suggesting potential differences between individual PARP inhibitors.

Balancing Efficacy and Safety

The improved disease control with PARP inhibitors came with increased toxicity. Patients receiving these agents experienced a significantly higher incidence of grade three or higher adverse events compared with non-PARP regimens. These findings underscore the importance of balancing PFS gains against the risk of serious side effects when selecting maintenance therapy. Clinicians are advised to carefully consider patient selection, monitor closely for toxicity, and engage in shared decision-making with patients.

Overall, the meta-analysis reinforces PARP inhibitors as an effective maintenance option for advanced ovarian cancer while highlighting the need for personalized treatment strategies.
