Cervical cancer is predominantly caused by persistent infection with high-risk HPV strains, particularly HPV16 and HPV18, which account for approximately 70% of cases. Dr. Kathy MacLaughlin, a family physician specializing in cervical cancer prevention at Mayo Clinic, emphasizes that while HPV screening is crucial, many patients face barriers with traditional speculum-based examinations, leading to lower screening rates in certain populations. To address these healthcare disparities, Mayo Clinic will soon offer an FDA-approved self-collection HPV testing option, making it one of the few medical institutions in the United States to provide this alternative.

Beginning February 5, Mayo Clinic’s Midwest locations will implement this innovative screening approach, with subsequent expansion to Arizona and Florida campuses. Unlike traditional screening that requires clinician-collected cervical samples for Pap smears and/or HPV testing, the self-collection method allows patients to privately obtain vaginal samples using a specially designed device. Dr. MacLaughlin explains that this approach eliminates the need for speculum insertion while maintaining testing accuracy, particularly benefiting individuals with disabilities, mobility issues, cultural or religious concerns, or trauma histories.

​How Self-Collection Testing Works​

The self-collection device resembles a tampon in its insertion method, with plastic wings controlling insertion depth to prevent discomfort. Users push a plunger to extend a soft, dry bristle brush into the vagina, then rotate the handle five times until audible clicks confirm proper sample collection. After retracting the brush and sealing the device, the sample is sent to a laboratory for analysis. This entire process occurs privately within a healthcare setting, ensuring professional oversight while maximizing patient comfort.

​Screening Accessibility and Future Directions​

Currently, self-collection screening must be conducted in clinical environments, though Dr. MacLaughlin anticipates future approval for home-based testing. She stresses that regardless of the method chosen, the primary goal remains early detection of precancerous conditions when treatment is most effective. By discussing options with healthcare teams, patients can select the screening approach that best addresses their needs while maintaining preventive care standards.
