The international multicenter AQUILA trial enrolled 390 high-risk SMM patients, randomly assigning them to receive either daratumumab or undergo active surveillance. After a median follow-up period of 65.2 months, the daratumumab group showed a remarkable 51% reduction in disease progression or death risk. At the five-year mark, 63.1% of daratumumab patients remained progression-free compared to 40.8% in the active monitoring group. Overall survival rates also significantly favored the treatment group, with 93% survival versus 86.9% in the control group.
