While most advanced cases cannot be cured, treatments can slow cancer growth, reduce symptoms, and extend life. Recent advances in diagnostic techniques and new therapies have improved survival rates. “Multiple treatments, including hormone therapy, can control the disease longer even when metastatic,” says Dr. Oliver Sartor, a Mayo Clinic oncologist specializing in prostate cancer.
Personalized Treatment Approaches
Prostate cancer treatment is highly individualized, differing significantly from person to person. “Prostate cancer exhibits remarkable diversity. Your neighbor’s treatment may differ from yours,” Dr. Sartor explains. After a biopsy confirms cancer, laboratory tests analyze cancer cells to determine their aggressiveness. Genetic testing may identify eligibility for targeted therapies or immunotherapy. Treatment plans also consider the cancer’s spread location—whether to bones, lymph nodes, or recurring after surgery—each requiring tailored strategies.
PSA Testing for Early Detection
The PSA blood test measures prostate-specific antigen levels, helping detect prostate cancer early when treatment is most effective. While elevated PSA may indicate cancer, it can also result from non-cancerous conditions like prostate enlargement. “PSA testing remains controversial as it may detect slow-growing cancers needing no immediate treatment, but it reduces late-stage diagnoses,” notes Dr. Sartor. Active monitoring, including regular PSA tests and imaging, may suffice for low-risk cases. Men at higher risk (e.g., African American men or those with family history) should consider earlier screening.
New Hope with Radiopharmaceuticals
Hormone therapy blocking testosterone is standard for advanced prostate cancer. When cancer becomes resistant to such treatments (castration-resistant prostate cancer), new options like PSMA lutetium-177 offer promise. This radioactive drug targets prostate cancer cells expressing PSMA protein. Eligibility requires prior hormone therapy and chemotherapy, plus a PSMA PET scan confirming high PSMA uptake and no liver involvement. “About one-third of patients respond well, one-third do not, and the rest show intermediate results,” says Dr. Sartor. Ongoing research may expand its use to pre-chemotherapy patients.
