Women working in emotionally demanding jobs with little workplace support face a significantly higher risk of developing type 2 diabetes, new European research has revealed.
A major Swedish study, published in Occupational & Environmental Medicine, found that women in roles requiring frequent emotional labor and public interaction face a 47% greater risk of developing type 2 diabetes when they also lack social support at work.
The research analyzed data from the Swedish Work, Illness, and Labour-Market Participation (SWIP) cohort, tracking around 3 million workers aged 30 to 60 over a 15-year period, from 2005 to 2020. More than 216,000 participants developed type 2 diabetes, with 60% of cases occurring in men.
However, the study uncovered that emotional demands pose a particularly strong threat to women’s metabolic health.
Researchers examined three aspects of person-contact roles:
- General contact with people (patients, customers, clients, or students)
- Emotional demands from helping people with serious problems
- Frequency of confrontational situations
Key findings include:
- Women exposed to high emotional demands had a 24% higher risk of type 2 diabetes.
- This risk increased to 47% when combined with low workplace support from colleagues or supervisors.
- Men also faced elevated risk: 20% higher with emotional demands and 15% higher with confrontational roles.
- These associations were stronger in both genders when social support at work was low.
The study found that general contact with people only increased diabetes risk in women when their jobs offered low levels of control, suggesting that autonomy at work may buffer some health impacts.
Women in professions such as healthcare, education, social work, and customer service—where emotional labor is high—may be particularly vulnerable. Researchers noted these roles often require workers to manage or suppress their genuine emotions to meet social and professional expectations.
“Healthcare professionals and social workers often take on others’ suffering without receiving emotional reciprocity,” the authors wrote. This emotional mismatch may increase chronic stress levels.
The biological link may involve chronic stress affecting the neuroendocrine system, resulting in elevated cortisol, insulin resistance, and decreased insulin production—factors known to contribute to type 2 diabetes.
Those who developed diabetes during the study were more likely to be older, born outside of Sweden, have lower education levels, and have reduced job control compared to healthier peers.
The study reinforces the importance of emotional support and autonomy in the workplace—not only for mental well-being, but also to prevent chronic diseases like diabetes.
