ADHD diagnoses are rising worldwide, driven largely by increased awareness and evolving diagnostic criteria. In the United States, 11% of children were diagnosed with attention-deficit hyperactivity disorder (ADHD) last year, the highest rate ever recorded. In the UK, new ADHD diagnoses have doubled among boys and quadrupled among girls over the past two decades.
A recent analysis in Nature examined the causes of this surge through interviews with scientists and a review of research studies. The journal highlighted that heightened awareness has led more individuals to suspect they have ADHD and seek evaluations. A pediatric neurologist at Ohio Rainbow Children’s Hospital explained, “The surge in ADHD diagnoses is less about a true increase in cases and more about changes in awareness and diagnostic criteria.”
Some experts warn of potential overdiagnosis. UC Berkeley professor Steven Hinshaw noted, “There are cases where individuals receive an ADHD diagnosis after just 15- to 20-minute online consultations. Poor or excessive diagnoses may be on the rise.”
Changes in diagnostic standards have also contributed. The age for symptom onset has been extended from 7 to 12 years, and adult criteria have been adjusted, broadening the pool of eligible patients.
Despite rising diagnosis rates, studies suggest the actual prevalence of ADHD has remained relatively stable. A recent global study found childhood ADHD prevalence at around 5.4% and adult prevalence at approximately 2.6%, consistent with figures from 10 to 20 years ago.
Scientists are increasingly exploring “biomarkers” to improve diagnosis and treatment. Traditionally, ADHD diagnoses rely on interviews, questionnaires, and observations, which can vary depending on clinicians and patient responses. Researchers are now investigating objective indicators such as blood tests, brain imaging, and brainwave tracking. Nature noted, “If biomarker-based diagnosis becomes widespread, ADHD could be identified more accurately in the future, enabling personalized medication for individuals.”
