Targeted Androgen Receptor Therapy Shows Promise in Advancing Modern Baldness Treatment

Hair loss, particularly androgenetic alopecia—commonly known as male or female pattern baldness—can take a significant emotional toll. Often beginning subtly in one’s 20s or 30s, it gradually progresses, impacting self-confidence and overall quality of life.

Patients frequently express frustration over limited treatment options beyond the well-known staples, minoxidil and finasteride. However, recent advancements in dermatology are bringing new hope. One such promising option is clascoterone, a topical treatment originally developed for acne, now showing encouraging potential for hair regrowth in pattern hair loss.

Clascoterone, currently approved in a 1% cream form (Winlevi) for acne, acts as a topical androgen receptor inhibitor. It works by competitively binding to androgen receptors in the skin and hair follicles, blocking dihydrotestosterone (DHT)—the hormone chiefly responsible for follicle miniaturization in androgenetic alopecia. By targeting DHT locally on the scalp, clascoterone helps prevent follicles from producing thinner, shorter hairs without significant systemic absorption, reducing the risk of hormonal side effects.

Clascoterone vs. Minoxidil

To understand clascoterone’s potential, it’s helpful to compare it with minoxidil. Widely available over the counter in 2% and 5% formulations, minoxidil was initially an oral antihypertensive medication discovered to promote hair growth as a side effect. It primarily acts as a potassium channel opener, increasing blood flow to hair follicles and potentially prolonging the hair growth phase. Minoxidil can enhance follicle size, stimulate dormant follicles, and influence growth factors like VEGF.

However, minoxidil does not address the hormonal cause of pattern baldness. While it can increase hair density in 40–60% of users, results are often modest, and continuous application is required; stopping treatment frequently leads to rapid hair loss. Side effects, including scalp irritation and unwanted facial hair growth, are common.

Clascoterone, by contrast, targets the underlying androgen-mediated follicle damage. This makes it potentially more effective for hormone-sensitive cases and complementary to minoxidil, which enhances follicle vitality through a different mechanism. Early studies suggest clascoterone promotes dermal papilla cell activity and keratinocyte proliferation, similar to minoxidil, but via a distinct pathway—raising hopes for synergistic effects when used together.

Promising Clinical Results

The latest evidence comes from December 2025 topline results of two large phase 3 trials (SCALP-1 and SCALP-2) by Cosmo Pharmaceuticals, the largest program ever conducted for a topical hair loss treatment. Over 1,465 men with androgenetic alopecia participated, applying a 5% clascoterone solution twice daily for six months. Both trials reported statistically significant improvements in target-area hair count, with one study showing a 539% relative improvement over placebo and the other 168%. Patients also reported visible hair growth and increased satisfaction, while the safety profile mirrored placebo, with no systemic hormonal changes—a key advantage over oral anti-androgens like finasteride.

Clascoterone represents the first novel topical mechanism for androgenetic alopecia in over three decades, addressing a critical gap for patients seeking non-systemic alternatives or those intolerant to oral treatments. While current phase 3 data focus on men, earlier studies indicate potential benefits for women with pattern thinning.

Important Considerations

Despite its promise, clascoterone is not a universal solution. It is designed for early to moderate hormonal pattern thinning and is unlikely to help with conditions like telogen effluvium, nutritional deficiencies, alopecia areata, or scarring alopecias. Accurate diagnosis, often through trichoscopy, patient history, and sometimes blood tests, is essential to determine suitability.

The 5% formulation for hair loss is pending regulatory approval, with Cosmo Pharmaceuticals completing 12-month safety follow-ups and FDA and EMA submissions expected in 2026. Until then, evidence-based treatments such as minoxidil, finasteride for eligible patients, and low-level laser therapy remain the frontline options.

As research continues to advance, targeted and safer hair loss treatments are emerging, offering renewed hope for patients. Those noticing thinning should consult a dermatologist promptly and avoid self-medicating.
