Research Shows Combination Pills Improve Hypertension and Cut Cardiovascular Risk

Single-pill combination medications (SPCMs), also known as fixed-dose combinations, help adults lower blood pressure faster and more effectively than taking multiple medications separately, according to a December statement from the American Heart Association published in Hypertension.

“Most people with high blood pressure need two or more medications to reach target blood pressure levels, but taking multiple pills daily can be confusing or hard to maintain,” said Jordan B. King, PharmD, MS, chair of the statement writing group and associate professor of population health sciences at the University of Utah. “Single-pill combination pills are valuable tools. Patients taking one combination pill achieve optimal blood pressure sooner than those taking the same medications separately.”

SPCMs combine two or more active ingredients in a single pill. The statement notes that only about 25% of Americans maintain blood pressure below 130/80 mm Hg, while roughly 122 million adults—about half of the U.S. population—have high blood pressure. The authors reviewed multiple studies supporting the effectiveness of SPCMs, including SPRINT, the National Health and Nutrition Examination Survey, and the ACCOMPLISH trial.

A January 2021 analysis found that SPCMs significantly improved blood pressure control, with modest but statistically meaningful reductions in both systolic and diastolic blood pressure at 12 weeks compared with taking separate pills. Despite these benefits, SPCMs use declined from 35.8% in 2009–2012 to 27.8% in 2017–2020, according to the National Health and Nutrition Examination Survey.

The statement highlights several barriers to SPCM use, including gaps in patient knowledge, insurance coverage and reimbursement policies, social determinants of health, prescriber practices, and regulatory or commercial availability. Authors suggest that SPCMs deliver the greatest clinical benefit when used to appropriately intensify therapy for patients with high blood pressure.
