Aleix Prat, Director of the Clínic Barcelona Comprehensive Cancer Center at Barcelona Clinical Hospital, shared new findings on LinkedIn from his team’s latest study published in Clinical Cancer Research.
The study, titled “Associations of the HER2DX Genomic Test with Biological and Pathological Features in HER2-Positive Breast Cancer,” analyzed 410 tumors across multiple centers to evaluate how the three HER2DX scores—pCR likelihood, relapse risk, and ERBB2 mRNA expression—correlate with standard histopathology in real-world clinical practice.
Key Findings
The HER2DX pCR score was linked to all eight pathological and immune features analyzed, including grade, hormone receptor status, HER2 IHC, Ki67, tumor-infiltrating lymphocytes (TILs), tertiary lymphoid structures (TLS), immune patterns, and histology.
Among 250 patients treated with neoadjuvant trastuzumab-based therapy, the HER2DX pCR score was the only independent predictor of pathological complete response (pCR), outperforming conventional clinical and pathological variables.
The relapse-risk and ERBB2 scores captured critical biological dimensions—proliferation, immune infiltration, luminal differentiation, and HER2 signaling—that are not fully reflected by standard pathology.
Prat emphasized that HER2DX provides clinically relevant information beyond what conventional pathology can offer. He credited breast cancer pathologist Esther Sanfeliu Torres for leading the work, alongside Anabel Martínez Romero and the broader team, and expressed gratitude to all collaborators, institutions, and patients involved in the study.
