Hearing Treatment Cuts Elderly Dementia Risk by Two-Thirds

New research shows that cochlear implants can dramatically reduce the risk of dementia in patients with severe hearing loss. Patients who received cochlear implants were found to have up to a two-thirds lower risk of developing dementia compared to those who did not undergo the surgery. This is the first large-scale domestic study to show that cochlear implants provide substantial brain health benefits beyond improving hearing.

The study was led by Prof. Jang Young-soo of Sanggye Paik Hospital’s Department of Otolaryngology. His team analyzed national health data from 2010 to 2020, covering 391,195 patients with moderate or severe hearing loss who qualified for disability registration. The research was conducted in collaboration with teams from Wonju Severance Christian Hospital and Seoul St. Mary’s Hospital.

All study participants had no prior history of dementia at the time of hearing loss diagnosis. Among 5,814 patients who received cochlear implants, only 4.9% were later diagnosed with dementia. In contrast, 16.1% of the 385,381 patients without implants developed dementia—almost three times higher.

The protective effect of cochlear implants was observed across age groups. Among patients aged 50 and older, 11.2% of those with implants developed dementia, compared to 17.5% in the non-implant group. In patients aged 70 and above, the dementia rates were 18.4% for the implant group and 21.8% for those without implants. Cochlear implant recipients also experienced a delayed onset of dementia. On average, dementia developed 5.2 years after hearing loss diagnosis in the implant group, compared to just 1.6 years in the non-implant group.

The researchers explained that cochlear implants may reduce dementia risk by easing the brain’s cognitive load caused by hearing loss. Severe hearing loss forces the brain to expend extra energy to process sounds, leaving fewer resources for memory, judgment, and other cognitive functions. Cochlear implants help restore hearing and free up cognitive resources, potentially protecting brain function.

Prof. Jang emphasized, “Hearing loss is the leading modifiable risk factor for dementia and can be actively treated. Cochlear implant surgery is not just a tool for hearing—it is a treatment that protects cognitive function.” He added that the benefits were seen even in patients over 70, suggesting hearing treatment could be a new strategy to prevent dementia in older adults.

The team plans to investigate the specific biological mechanisms linking hearing restoration and cognitive function through future clinical trials. Their findings were published in the journal Otology and Neurotology.
