A new large-scale study suggests that physical frailty may play a direct role in increasing the risk of dementia. Published on September 17, 2025, in Neurology®, the journal of the American Academy of Neurology, the research highlights frailty as a potentially modifiable risk factor for dementia.
Physical frailty is characterized by the presence of three or more of these symptoms: frequent fatigue, low physical activity, slow walking speed, reduced grip strength, and unintentional weight loss. While previous studies linked frailty with cognitive decline, this research provides stronger evidence that frailty could contribute to dementia development rather than only serving as an early sign of the disease.
The study followed 489,573 participants from Zhengzhou University, China, with an average age of 57, over 14 years. Among them, 4.6% met the criteria for frailty, 43.9% were classified as pre-frail (1 or 2 symptoms), and 51.5% were not frail. During the study period, 8,900 individuals developed dementia.
Key findings include:
Individuals with frailty were nearly three times more likely to develop dementia compared to those without frailty.
Those classified as pre-frail had a 50% higher risk of dementia.
Participants with both frailty and genetic markers linked to dementia faced almost four times the risk compared to those without either risk factor.
Brain imaging and biological markers showed structural changes associated with dementia in frail individuals, suggesting a potential biological pathway from frailty to dementia.
Researchers accounted for confounding factors such as age, education, and physical activity, strengthening the connection between frailty and dementia risk. Interestingly, the study also found little evidence that dementia increases the risk of frailty, underscoring frailty’s possible causal role.
Yacong Bo, PhD, lead author from Zhengzhou University, emphasized the importance of these findings for public health: “These findings reinforce the importance of identifying and managing frailty as a strategy for preventing dementia.”
However, the study’s reliance on self-reported data for most frailty symptoms is a noted limitation, which could affect the accuracy of symptom reporting.
This research received funding from multiple Chinese health and scientific organizations, including the Chinese National Health Commission Key Laboratory of Birth Defects Prevention.
