Creatine is commonly known for its use in improving athletic performance, but it also plays a crucial role in maintaining energy balance in the brain. Low levels of creatine in the brain have been associated with depressive symptoms, and some studies suggest that higher dietary creatine intake may be linked to a lower prevalence of depression. However, despite these findings, creatine is not yet recommended as a treatment for depression by clinical guidelines.
Research Objectives and Methodology
The study aimed to evaluate the effectiveness of creatine supplementation on depressive symptoms by analyzing randomized controlled trials (RCTs). The researchers searched databases like Cochrane Library, Web of Science, Medline, and Embase for relevant studies. These studies included both creatine and placebo groups, with varying co-interventions like exercise, psychotherapy, or antidepressants.
Study Design and Statistical Approach
All trials included in the study had a parallel-group design, comparing the effects of creatine versus a placebo. The median dose of creatine was 5 grams per day, though some studies varied in dosing. Meta-analysis was conducted using random-effects models, and the risk of bias was assessed using the Cochrane Risk of Bias tool.

A total of 558 records were identified, and after screening, 34 studies were fully analyzed. From these, 11 studies met the criteria for inclusion in the final analysis. However, the heterogeneity among the studies was high, and the quality of evidence was rated as very low according to the GRADE criteria. Some trials reported minor gastrointestinal issues and slight increases in serum creatinine levels, though these were not severe.
Subgroup and Bias Analyses
Further analyses revealed that the positive effects of creatine were mainly driven by studies with a high risk of bias. Higher-quality studies with a lower risk of bias showed no significant effect. When the researchers adjusted for potential publication bias, the positive effect was reduced to null, suggesting that the observed benefit may have been overstated.
Summary of Findings and Implications
The study found a statistically significant reduction in depression symptoms with creatine, but the effect was too small to be clinically meaningful. The reduction in depression scores was just 2.2 points, which is below the threshold for clinical significance.
