A growing body of research continues to reveal the intricate connections between the brain and body, and a new study adds to that understanding. Children with autism spectrum disorder (ASD) are significantly more likely to experience ongoing gastrointestinal (GI) issues than their typically developing peers. These stomach and digestive problems are not only frequent but often chronic—sometimes persisting throughout childhood—and appear to correlate with other behavioral and developmental challenges.
The study, published in the journal Autism, found that nearly half of all children with autism experienced at least one period of GI symptoms, while around 30% reported continuous issues across multiple developmental stages. In contrast, more than 60% of children without autism showed no GI problems during the study period.
Long-Term Study Tracks Symptoms From Early to Middle Childhood
Researchers from the University of California, Davis MIND Institute conducted one of the most comprehensive analyses to date on gastrointestinal health among children with autism. The study followed 475 children—322 diagnosed with autism and 153 without—from the ages of 2 to 12.
Developmental and behavioral pediatricians interviewed families at multiple intervals using a detailed gastrointestinal history questionnaire. This longitudinal approach allowed researchers to assess symptom patterns and persistence over time, offering a rare view into how GI issues evolve alongside developmental changes.
According to the findings, children on the autism spectrum not only had more GI symptoms at every stage but were also more likely to experience multiple symptoms simultaneously, such as constipation, abdominal pain, bloating, and diarrhea. Interestingly, in many cases, no clear medical cause could be identified for these symptoms, suggesting that underlying neurological or sensory factors might contribute to the problem.
Impact Beyond the Digestive System
The study’s authors emphasized that gastrointestinal discomfort in children with autism is rarely an isolated issue. Persistent GI problems were linked with a range of co-occurring challenges, including sleep disturbances, heightened anxiety, irritability, sensory sensitivities, and repetitive behaviors.
“This is a reminder that the brain and the body are deeply connected,” said Dr. Christine Wu Nordahl, senior author of the study and professor of psychiatry and behavioral sciences at the UC Davis MIND Institute. “When children are experiencing stomach pain or other GI problems, it doesn’t just affect digestion—it can influence sleep, mood, and how they interact with the world around them.”
The findings underscore the need for a more holistic approach to autism care, where physical and behavioral health are addressed together rather than in isolation.
Parents’ Observations Validated
For many families, these results provide scientific validation for what they have long observed in their children. “For parents, this study confirms what many have been saying for years: that GI symptoms are very common in children with autism,” said Dr. Bibiana Restrepo, developmental pediatrician at UC Davis and lead author of the study.
Restrepo added that pediatricians should routinely ask about gastrointestinal health during medical visits. “Clinicians should ask about these issues in routine visits, because effective treatment may help children feel better and thrive,” she said.
Toward More Collaborative Care
The researchers hope their findings will encourage greater collaboration among healthcare providers—including pediatricians, gastroenterologists, and autism specialists. Understanding the overlap between physical symptoms and behavioral manifestations could lead to better outcomes and improved quality of life for children with autism.
“Supporting gastrointestinal health is one important step toward improving overall quality of life for children with autism,” Nordahl emphasized.
The study also opens the door to future research exploring why children with autism are more prone to gastrointestinal distress. Potential factors under investigation include differences in gut microbiota, diet, sensory sensitivities, and neurological processing of pain and discomfort.
