The NHS has today unveiled the first wave of neighbourhood health services, designed to bring comprehensive care closer to home in 43 high-need communities across England. The new programme, backed by a £10 million investment, relies on local GP-led teams working collaboratively with social care, mental health, dental, pharmacy, voluntary sector partners, and others to deliver a seamless end-to-end patient experience.
A Community-Centred Care Revolution
These teams will address both immediate medical needs—such as prescribing antibiotics—and broader determinants of health by offering weight management, health coaching, and tailored lifestyle interventions, particularly benefiting those living with long-term conditions like diabetes, high blood pressure, arthritis, MS, or epilepsy.
Introduced under the health secretary’s “Plan for Change,” the initiative aims to ease patients’ journeys through the healthcare system and reduce unnecessary hospital admissions.
Data from pilot studies indicate promising outcomes: one London project cut A&E attendances by 7% and hospital admissions by 10%; in Cornwall, unplanned GP visits fell by 7%, vaccination rates rose 47%, and cancer screening and health checks jumped 82%.
Scale, Strategy, and Support
Depending on their choice, GP practices can join as a single-neighbourhood provider serving roughly 50,000 people, or as part of a multi-neighbourhood provider catering to about 250,000 individuals. Care plans will be coordinated using the NHS App, facilitating efficient communication and continuity.
Each of the 43 regions will appoint a programme lead to collaborate with local services and design area-specific delivery models. GPs will receive support through new contract frameworks from 2026, national coaching, knowledge-sharing workshops, and access to expert-support hubs.

Local MP Paul Waugh (Rochdale) hailed the programme as a vital opportunity for better, joined-up care in one of the country’s most inequality-affected regions.
The NHS Confederation highlights the importance of local integration across health, social care, and community sectors, and notes ongoing efforts—such as data, workforce, finance, and estate reform—to remove barriers to implementation.
