Study Link Between Persistent Fatigue and Inflammation in Breast Cancer Survivors

A new study from George Mason University’s College of Public Health has found that lingering fatigue among breast cancer survivors may be closely tied to increased inflammation, even during low-stress activities.

Lead researcher Ali Weinstein, professor of global and community health and senior scholar at the Center for the Advancement of Well-Being, examined how survivors responded to both mentally and physically demanding tasks.

The team measured inflammation markers alongside participants’ self-reported fatigue—two symptoms often seen in breast cancer survivors and suspected to affect quality of life and potentially influence cancer recurrence.

Results revealed that women who reported higher fatigue levels at the start of the study showed greater increases in inflammatory markers—specifically TGF-β and eotaxin—after completing tasks. Notably, even participants in the control group, who simply watched a nature video, experienced elevated inflammation and fatigue when they had high baseline fatigue.

Unexpectedly, most participants showed no significant changes in inflammation or fatigue after either mentally or physically fatiguing tasks, suggesting that brief challenges may not trigger widespread effects in all survivors.

Published in BMC Women’s Health, the study addresses three understudied areas: the link between mental fatigue and inflammation, short-term responses to fatigue-inducing tasks, and differences between physical and mental exertion.

Weinstein emphasized the importance of understanding these connections: “Past research has linked inflammation to cancer progression or recurrence. Fatigue deeply affects daily life, and we suspect it is tied to inflammation. We want to know how even short-term exposures could accumulate and worsen inflammation over time.”

Participants were randomly assigned to one of three activities—a brisk walk/run, a mentally challenging computer test, or watching a nature video—before having blood samples and fatigue ratings collected at three intervals: pre-task, post-task, and after a 30-minute recovery.

The findings highlight the potential need for personalized fatigue management strategies for breast cancer survivors, tailored to each individual’s baseline fatigue levels and inflammatory responses to everyday cognitive and physical demands.
