A new population-based study shows that a patient’s quality of life at the time of pancreatic cancer diagnosis can strongly predict survival. The research focused on people with non-metastatic advanced pancreatic cancer, including borderline resectable and locally advanced disease.
These two groups often vary widely in how their tumors behave and how they respond to treatment. The study aimed to determine whether patient-reported well-being at diagnosis could offer useful prognostic information.
Quality of Life as a Predictor
Researchers measured quality of life using the EORTC QLQ-C30 questionnaire when patients were first diagnosed. The study enrolled 230 patients between 2018 and 2020, and baseline data were available for 143 of them. Median overall survival for the group was 14 months.
Several aspects of quality of life were linked to survival. Emotional functioning and financial concerns were independent predictors. However, constipation stood out as the strongest indicator.
Patients who reported severe constipation had a median overall survival of 10 months. Those with mild or no constipation survived around 16 months. The hazard ratio for constipation was 1.95, meaning these patients faced almost twice the risk of death compared with those who had fewer symptoms.
Impact on Pancreatic Cancer Care
The study shows that structured quality-of-life assessments can play an important role in clinical care. A patient’s condition at diagnosis—especially the severity of constipation—may help doctors identify risk levels, choose treatments and plan supportive care.
Because treatment for borderline resectable and locally advanced pancreatic cancer often needs to be highly personalized, patient-reported outcomes can provide valuable guidance from the very beginning. Routine measurement of quality of life could also support more patient-centered care. It ensures that symptom relief, daily function and overall well-being remain core parts of treatment planning.
These findings add to growing evidence that symptoms once considered secondary can offer meaningful prognostic information in pancreatic cancer.
