A new study from the University of Southern California suggests that how blood pressure changes from heartbeat to heartbeat may be as important as the overall reading, especially for brain health in aging adults.
The researchers found that older adults who experience larger beat-to-beat swings in blood pressure tend to have smaller brain volumes in regions tied to memory and higher levels of a protein associated with nerve-cell injury.
Even when blood pressure is well controlled with medication, rapid fluctuations from beat to beat are linked with poorer memory and signs of brain shrinkage and injury, said Daniel Nation, a professor at USC and the study’s senior author. He told Fox News Digital that the work points to a broader pattern: blood pressure is not static; it adapts to the body’s needs, but aging can make that regulation less precise.
Study design and key measurements
The study enrolled 105 adults aged 55 to 89.
Researchers monitored blood pressure continuously for several minutes while participants underwent brain scans.
They assessed arterial stiffness and how much blood pressure changed between heartbeats.
Findings show that among those with similar average blood pressure, the most erratic readings—particularly alongside stiffer arteries—correlated with signs of brain aging. Participants with greater variability had smaller hippocampal and entorhinal cortex regions, areas among the first affected in Alzheimer’s disease. They also exhibited higher levels of neurofilament light, a biomarker of nerve-cell damage.
Implications for risk and treatment
The results held after adjusting for age, sex, and average blood pressure, suggesting that fluctuations themselves may be a risk factor for brain changes.
The study underscores that preventing brain injury and memory decline may require attention to moment-to-moment blood pressure stability, not just to lowering average levels.
Caveats and next steps
Experts caution that the research shows correlations in a cross-sectional snapshot and does not prove causation.
Nation emphasized the need for longitudinal studies to determine whether rapid blood pressure fluctuations predict future brain shrinkage and cognitive decline.
There are currently no therapies specifically targeting these fluctuations. However, maintaining regular blood pressure control remains important, as does adherence to prescribed medications to reduce the risk of brain injury and memory decline.
Clinical and public health relevance
This work adds a new dimension to how clinicians assess cardiovascular risk and its impact on the brain. It suggests that even people with normal or well-controlled blood pressure could be at risk if their readings fluctuate markedly over short periods. The authors advocate prioritizing research into therapies that address rapid blood pressure changes, which are not fully addressed by current treatments focused on lowering average blood pressure.
