uman papillomavirus (HPV) causes about 70% of head and neck cancers in the U.S., and their incidence continues to rise yearly. Unlike cervical cancer, which benefits from routine screening, there is currently no reliable test to detect HPV-related head and neck cancers before symptoms appear. Most diagnoses occur after tumors have significantly grown, spreading to lymph nodes and causing noticeable symptoms, which complicates treatment.
Now, researchers at Mass General Brigham have developed a promising liquid biopsy test, called HPV-DeepSeek, capable of detecting HPV-driven head and neck cancers up to a decade before symptoms develop. This early detection could transform cancer screening and treatment by enabling earlier interventions and improving survival and quality of life.
How HPV-DeepSeek Works
HPV-DeepSeek uses whole-genome sequencing technology to identify tiny fragments of HPV viral DNA shed by tumors into the bloodstream. Previous studies by this team showed the test to be 99% sensitive and specific when detecting cancers at diagnosis, outperforming existing diagnostic tools.
The latest research, published in the Journal of the National Cancer Institute, analyzed 56 blood samples from the Mass General Brigham Biobank. Of these, 28 samples were from individuals who later developed HPV-linked head and neck cancer, while 28 were controls from healthy individuals.
The blood test detected HPV tumor DNA in 22 of the 28 cancer cases well before symptoms surfaced. All control samples tested negative, confirming the test’s high specificity. Findings showed the closer the sample was collected to diagnosis, the more likely HPV DNA was detected, with one positive result from nearly eight years before diagnosis.
By applying machine learning to refine the test, researchers improved detection to 27 out of 28 cases—accurately identifying HPV DNA up to 10 years ahead of clinical diagnosis.
Implications for Early Screening and Treatment
Dr. Daniel L. Faden, lead author and head and neck surgical oncologist, highlighted the significance of early detection: “When patients present with symptoms, the cancer has often advanced, requiring aggressive treatments with lasting side effects. With HPV-DeepSeek, we can potentially identify cancers at their earliest stages, reducing the need for harsh therapies and improving patient quality of life.”
The ability to detect HPV-related cancers a decade in advance opens new avenues for earlier intervention strategies, surveillance, and individualized patient care. It also addresses a critical gap since no screening test currently exists for these cancers, unlike cervical cancer.
What This Means for Patients
This liquid biopsy approach offers a non-invasive, highly accurate way to identify at-risk individuals years before cancers develop symptoms. Early diagnosis could translate to more effective, less invasive treatments and better overall outcomes for patients facing HPV-related head and neck cancers.
Ongoing research will focus on validating HPV-DeepSeek in larger, diverse populations and exploring how best to integrate it into routine cancer screening protocols.
