In vitro fertilization (IVF) is one of the most effective fertility treatments available today. Compared to other fertility methods or natural conception, it often offers a shorter path to pregnancy. IVF involves a series of carefully coordinated steps carried out by doctors, nurses, embryologists, lab specialists, and patients themselves.
Although the process can seem overwhelming at first, it can be broken down into four main stages:

Step 1: Ovarian Stimulation
During this stage, medications—usually follicle-stimulating hormone (FSH) and sometimes luteinizing hormone (LH)—help the ovaries produce more eggs than usual. The goal is simple: increase the number of eggs available, since not every egg will mature, fertilize, or develop into a healthy embryo.
Research shows that retrieving around 15–20 eggs gives the best chance of success, without significantly increasing risks. However, too much stimulation can lead to ovarian hyperstimulation syndrome (OHSS), which ranges from mild bloating to, in rare cases, more serious complications. Careful monitoring and tailored medication plans reduce these risks significantly.
Timeline: Stimulation usually begins on day 2–4 of the menstrual cycle and lasts 8–12 days. Patients are monitored closely with ultrasounds and blood tests before receiving a “trigger shot” to prepare the eggs for retrieval.
Step 2: Egg Retrieval
About 35 hours after the trigger shot, eggs are collected through a short procedure under anesthesia. Using ultrasound guidance, a thin needle removes the fluid from ovarian follicles, which contains the eggs. The process takes about 15–20 minutes.
Patients usually go home the same day, experiencing only mild cramping. Before leaving, they’ll learn how many eggs were retrieved. At this point, partners or donors provide sperm, either fresh or previously frozen, for fertilization.
Step 3: The Embryology Lab
This is where fertilization and early embryo development happen. Two common fertilization methods are used:
- Conventional IVF: Sperm are placed in a dish with eggs, allowing fertilization to occur naturally.
- Intracytoplasmic sperm injection (ICSI): A single sperm is injected directly into an egg, often recommended for male-factor infertility.
After fertilization, embryos develop in special incubators for up to seven days, until they reach the blastocyst stage.
Step 4: Embryo Transfer
The embryo transfer is a quick, nearly painless procedure similar to a Pap test. A thin catheter is used to place the embryo(s) into the uterus under ultrasound guidance. This can be done with a fresh transfer (a few days after retrieval) or a frozen transfer (at a later cycle, after embryos are frozen and thawed).
After transfer, patients usually rest briefly before going home. A blood test about two weeks later will confirm whether implantation was successful.
The Two-Week Wait
The period after embryo transfer can be emotionally challenging. During this time, patients continue medications such as estrogen and progesterone to support implantation. Mild cramping, spotting, or bloating are common. A blood test typically occurs 12–14 days after retrieval to confirm pregnancy.
