AI Could Improve IVF Success Rates

In vitro fertilization (IVF) has given hope to millions of couples experiencing infertility, but the procedure remains costly, time-consuming, and emotionally challenging. Now, researchers are exploring how artificial intelligence (AI) can enhance the success of IVF by helping doctors make more precise decisions during treatment.

Understanding the IVF Process

During IVF, doctors monitor ovarian follicles—small sacs in the ovaries that contain eggs—using ultrasound scans. The timing of the hormone “trigger” injection, such as human chorionic gonadotropin (hCG), is critical. This injection prepares eggs for retrieval and fertilization. Administering it too early or too late can reduce the number of mature eggs collected, which may impact the likelihood of pregnancy.

After egg retrieval and fertilization, an embryo is selected for implantation in the uterus, with the goal of achieving a successful pregnancy.

The Role of AI in IVF

Researchers at Imperial College London, in collaboration with the University of Glasgow, University of St Andrews, and Imperial College Healthcare NHS Trust, analyzed data from more than 19,000 IVF patients using “Explainable AI.” This form of AI allows clinicians to understand the reasoning behind its recommendations, making it a practical tool for healthcare decisions.

The study examined which follicle sizes were most likely to result in mature eggs and ultimately live births. It found that follicles measuring 13–18mm on the day of the trigger injection were associated with higher rates of mature egg retrieval and improved chances of pregnancy.

Moving Toward Personalized IVF

Currently, clinicians often base the trigger shot on the size of the two or three largest follicles, typically those above 17–18mm. The new findings suggest that evaluating the full range of follicle sizes could optimize egg retrieval and improve IVF outcomes. By focusing on a greater proportion of intermediately sized follicles, doctors can tailor treatment to the individual patient’s ovarian response.

“IVF produces a vast amount of data, and it can be difficult for doctors to fully utilize it in real-time decision-making,” said Dr. Ali Abbara, co-senior author of the study and Consultant in Reproductive Endocrinology at Imperial College Healthcare NHS Trust. “AI provides a new paradigm for personalizing IVF, improving clinical outcomes, and giving each couple the best chance of having a baby.”

Understanding Follicle Sizes and Hormone Levels

The study highlighted the importance of intermediately sized follicles (13–18mm). Stimulating ovaries for too long can result in excessive larger follicles (>18mm), which may prematurely raise progesterone levels. High progesterone can negatively affect the uterine lining, reducing the chance of embryo implantation and successful pregnancy. AI can help clinicians balance follicle stimulation to avoid these issues.

Future Directions

The research team aims to develop an AI tool that integrates these findings to assist clinicians in real-time decision-making during IVF. They plan to seek funding to bring this tool into clinical trials, potentially transforming how fertility treatments are delivered.
