A common, decades-old blood pressure medication may help preserve the body’s natural insulin production in adults newly diagnosed with type 1 diabetes, according to new research. The findings offer a glimmer of hope for a future where managing the autoimmune condition could be simpler and more effective.
The study, known as the Ver-A-T1D trial, was presented at the European Association for the Study of Diabetes (EASD) annual meeting. It investigated the effects of slow-release verapamil (360mg daily) on beta-cell function. These insulin-producing cells in the pancreas are mistakenly destroyed by the immune system in type 1 diabetes.
Study Design and Key Findings
The European multicenter trial included 136 adults across six countries. Participants were randomly assigned to receive either verapamil or a placebo for 12 months.
The primary goal was to measure the preservation of beta-cell function using a stimulated C-peptide test. While the initial analysis narrowly missed its statistical target, a secondary “per-protocol” analysis (which included only participants who followed the study plan perfectly) showed a statistically significant positive effect, suggesting a real biological benefit from the drug.
Secondary Benefits and Safety Profile
Secondary outcomes were also encouraging:
Improved Blood Sugar Control: Participants taking verapamil showed improved HbA1c levels (a key measure of long-term blood sugar control) during the first six months of treatment.
Comparable Safety: The drug demonstrated its well-known safety profile with no unexpected side effects. The most common side effects were mild, reversible heart rhythm changes (first-degree AV block and bradycardia), which were managed with dose reduction.
A Challenge and the Path Forward
A key challenge was that the disease progressed much more slowly in the untreated placebo group than researchers had anticipated. This made it harder to detect a clear treatment effect within the study’s timeframe.
“The unique non-immunosuppressive mechanism, the established safety profile and cost-effective delivery make verapamil an attractive option,” said lead author Professor Thomas R. Pieber of the Medical University of Graz, Austria.
The research team is now analyzing 24-month follow-up data and has initiated an extension study to understand verapamil’s long-term potential. Future plans include testing verapamil in combination with immune-modulating therapies in a larger platform trial, with the ultimate goal of fully halting the progression of type 1 diabetes.

It is crucial to note that this is still a research drug for this specific use. Patients should not ask their doctors for verapamil to treat type 1 diabetes outside of a clinical trial. However, the results are a significant step toward repurposing an existing, safe, and affordable medication to improve lives and potentially change the course of a new type 1 diabetes diagnosis.
