A groundbreaking study has mapped exactly how much common blood pressure medications lower blood pressure, giving doctors a powerful new online tool to guide treatment and reduce reliance on trial-and-error prescribing.
High blood pressure, or hypertension, affects millions and is a major risk factor for heart attack and stroke. Until now, finding the most effective medication often involved trial and error, as doctors navigated dozens of drug options, doses, and combinations.
Led by researchers at the University of New South Wales’ George Institute for Global Health in Australia, the study analyzed data from 484 clinical trials involving over 100,000 participants. Researchers measured the effects of five main types of blood pressure medications—ACE inhibitors, ARBs, beta-blockers, calcium channel blockers, and diuretics—both alone and in combination.
Dr. Nelson Wang, the study’s lead author and cardiologist, emphasized the importance of even small reductions in blood pressure. “Every 1 mmHg drop in systolic pressure lowers your risk of heart attack or stroke by about 2%,” he said. “With so many drugs, doses, and combinations available, having a clear map of their effectiveness can save lives.”
Key findings include:
A single medication at a standard dose lowered blood pressure by an average of 9 mmHg. Doubling the dose added only about 1.5 mmHg more.
Two-drug combinations lowered blood pressure by about 15 mmHg, with doubled doses adding roughly 2.5 mmHg more.
Patients starting with lower blood pressure saw smaller reductions.
Most single drugs were “low-intensity” (less than 10 mmHg drop), while most two-drug combinations were “moderate-intensity” (10–19 mmHg). Only about 10% of combinations produced a “high-intensity” drop of 20 mmHg or more.
Using these insights, the team developed the Blood Pressure Treatment Efficacy Calculator, an online tool that predicts how different medications and combinations will affect an individual’s blood pressure. Early testing shows it accurately forecasts systolic blood pressure outcomes, offering doctors a more precise and personalized approach to hypertension management.
With hypertension often called a “silent killer,” this tool represents a major step forward in preventing heart attacks, strokes, and other complications, helping patients get the most effective treatment faster and safer.
