High-Dose Vitamin D in Pregnancy May Lower Childhood Wheeze Risk

A recent Cochrane systematic review suggests that high-dose vitamin D supplementation during pregnancy could significantly reduce the risk of childhood wheeze, an early indicator of asthma. However, evidence for administering vitamin D directly to infants remains inconclusive.

The review, published in the Cochrane Database of Systematic Reviews, examined 18 randomized controlled trials encompassing more than 10,000 participants.

Researchers analyzed the effects of vitamin D supplementation during pregnancy, breastfeeding, and early childhood on the subsequent development of asthma and wheezing.

Findings indicate that pregnant women taking high-dose vitamin D—ranging from 2,800 to 4,400 IU per day—experienced a 21% reduction in the risk of childhood wheeze compared to those receiving standard prenatal doses of 400 IU per day.

While trends suggested a potential reduction in formal asthma diagnoses, the evidence did not reach statistical significance.

Conversely, supplementation in infants and young children showed little to no effect. Analysis of three studies involving 588 children found no statistically significant reduction in asthma risk from any dose of vitamin D, and comparisons of high versus low doses yielded only very uncertain results. Some evidence suggested a modest decrease in early childhood airway infections, though this was rated as low certainty.

“High-dose vitamin D in pregnancy appears to play a protective role against childhood wheeze, although its impact on a confirmed asthma diagnosis remains unclear,” said the review authors. “Evidence for direct supplementation in infants is currently too limited to draw firm conclusions.”

Childhood asthma remains one of the most common chronic diseases worldwide, often persisting into adulthood and causing recurrent wheezing, coughing, and breathing difficulties.

Previous research has linked vitamin D deficiency to increased risks of allergic diseases, prompting investigations into whether supplementation during critical developmental windows could prevent long-term respiratory problems.

The Cochrane review represents the most comprehensive assessment to date, synthesizing data from seven major medical databases and following rigorous evidence-based standards. Its findings may help inform prenatal health recommendations and future research priorities aimed at preventing early-life respiratory illnesses.
