Arginine Toothpaste Significantly Reduces Cavities in Children, Study Finds

A new clinical trial shows that arginine-based toothpastes can significantly reduce dental caries in children, potentially offering an effective alternative to traditional fluoride toothpaste.

The study, published in JDR Clinical & Translational Research and announced by the International Association for Dental, Oral, and Craniofacial Research (IADR) and the American Association for Dental, Oral, and Craniofacial Research (AADOCR), found that higher-concentration arginine dentifrices reduced cavities as effectively—or even more effectively—than sodium fluoride formulations.

Dental caries remain a global oral health challenge, and while fluoride’s anticaries effects are well established, researchers are exploring additional strategies to enhance prevention.

The study, “Arginine Dentifrices and Childhood Caries Prevention: A Randomized Clinical Trial”, was led by Wei Lin of Sichuan University. Conducted over two years from April 2019 to March 2022, this phase III, double-blind, three-arm, randomized controlled trial involved 6,000 children aged 10–14 across three centers in China.

All participants had at least two active caries lesions and were assigned one of three dentifrices: 8.0% arginine, 1.5% arginine, or 0.32% sodium fluoride (NaF) as a positive control.

The primary outcomes measured were DMFS (decayed, missing, and filled surfaces) and DMFT (decayed, missing, and filled teeth) scores after two years of toothpaste use.

Results showed that the 8.0% arginine toothpaste reduced DMFS scores by 26.0% and DMFT scores by 25.3% compared with the sodium fluoride control, a statistically significant improvement. The 1.5% arginine toothpaste did not show a statistically significant difference from the fluoride control.

These findings confirm that higher-concentration arginine dentifrices can provide equal or superior protection against cavities in children with active caries, suggesting a promising alternative or complement to traditional fluoride toothpastes.
