Weighted Vests May Help Older Adults Preserve Bone Density During Weight Loss, Study Finds

Older adults aiming to lose weight without sacrificing bone or muscle mass may benefit from a simple intervention: wearing a weighted vest while staying physically active, according to new research published in Frontiers in Aging.

The peer-reviewed study, titled “Does time spent upright moderate the influence of a weighted vest on change in bone mineral density during weight loss among older adults,” suggests that time spent standing or stepping plays a critical role in whether weighted vests can help preserve bone mineral density during weight loss.

Addressing a Common Risk of Weight Loss

While weight loss offers significant health benefits, it can also lead to reductions in bone and muscle mass — a particular concern for older adults. Declines in bone mineral density increase the risk of fractures and disability, making it essential to find strategies that protect skeletal health during weight reduction.

Weighted vests are designed to offset lost body weight by adding an external load equivalent to the pounds shed. Researchers say this added load may help prevent metabolic slowdown and maintain muscle and bone strength, both of which are often compromised during dieting.

Study Design and Participants

The analysis was led by Jason Fanning, associate professor of health and exercise science, who examined data from Wake Forest University’s INVEST in Bone Health trial. The randomized controlled trial, led by Kristen Beavers, evaluated whether wearing a weighted vest could help preserve bone mineral density over one year of weight loss.

Participants were divided into three groups:

  • Weight loss only
  • Weight loss plus resistance training
  • Weight loss plus weighted vest

Those in the weighted vest group were instructed to wear the vest for at least eight hours per day.

Key Findings

  • Weighted vest plus weight loss: Participants who spent more time standing or stepping experienced positive changes in bone mineral density.
  • Weight loss alone: Increased time spent upright was associated with negative changes in bone mineral density.
  • Weight loss plus resistance training: Time spent upright did not significantly affect bone mineral density.

The findings indicate that simply wearing a weighted vest may not be sufficient. The benefits appear strongest when participants remain upright and active, exposing their bones to additional mechanical load.

Movement Enhances Effectiveness

Researchers believe that spending more time upright increases the amount of load applied to bones while wearing the vest, potentially counteracting bone loss commonly seen during weight reduction.

The positive changes observed in the weighted vest group have prompted the INVEST research team to design a follow-up study. The upcoming research will explore whether encouraging individuals to move more frequently can further enhance the vest’s ability to preserve bone mass.

“If we’re going to be putting vests on people, we need to train those people to be up and moving,” Fanning said.

Fanning is also a primary investigator in a separate study examining how movement influences arthritis-related pain.

Implications for Healthy Aging

The study highlights the importance of combining weight loss strategies with load-bearing activity to protect bone health in older adults. As researchers continue to refine practical interventions, weighted vests — paired with regular movement — may offer a promising approach to safer, more sustainable weight loss in aging populations.
