Carrying extra weight also challenges the cardiovascular system, enhancing heart health, while promoting balance and body awareness—critical factors in reducing falls and protecting joints, especially in older adults.

Falls are a growing concern in the U.S., with over 10,000 people turning 65 daily and fall-related injuries projected to rise from 52 million in 2018 to 73 million by 2030. Medical care costs for falls already reach approximately $50 billion annually (CDC). Weighted vests may offer a practical approach to mitigating these risks.

Evidence-Based Benefits

  • Improved Muscle Function: A 2002 study found older adults who wore a vest during a 12-week stair-climbing program improved leg muscle power and performance.
  • Potential Bone Health Benefits: A 1993 study reported modest bone density gains in older women wearing vests during low-level exercise. Another study in 2003 found significant hip bone density improvement when participants used weighted vests during 32 weeks of walking and strength training.
  • Metabolic Support: In a 2025 study, older adults wearing weighted vests for 10 hours per day while dieting regained less weight over the following year compared to those who dieted without vests, likely due to metabolic benefits from added resistance.

Weighted Vests and Bone Health

Weighted vests are not a cure-all, and their benefits vary by individual and context. In the INVEST in Bone Health clinical trial, 150 older adults with obesity participated in a 12-month weight loss program. Participants were divided into three groups: weight loss alone, weight loss plus eight hours daily weighted-vest use, and weight loss plus supervised resistance training.

While all groups lost approximately 10% of their body weight, declines in hip bone density (1.2–1.9%) occurred across the board. Weighted-vest use did not prevent hip bone loss compared with weight loss alone. However, both the vest and resistance-training groups showed increased markers of bone formation, suggesting potential long-term skeletal benefits.

Further analysis indicated that weighted vests may provide greater bone health advantages for women than men, and that standing more frequently while wearing the vest further supports bone strength. These findings highlight that the effectiveness of weighted vests can depend on individual factors and usage patterns.

Tips for Safe Use

Starting a new weight-bearing activity requires caution. Experts recommend a “start low and go slow” approach, particularly for those new to exercise or with medical conditions.

Weighted vests should fit comfortably, without interfering with posture, breathing, or stride. Red flags include hunching, clipped walking steps, low back pain, or hyperextension. Gradually increasing the weight can help maintain consistent muscle and bone stress. Most importantly, listen to your body—if you experience pain, remove the vest and seek professional guidance.

Weighted vests offer a practical, evidence-backed method to enhance everyday activity, improve strength, boost metabolism, and support balance. While they are not a replacement for structured exercise or medical advice, when used correctly, they provide a low-tech solution with potentially high-impact health benefits.
