Simon Cowell Opens Up About Botox, Fillers, and Controversial Treatments in His Plastic Surgery Journey

Simon Cowell, the music mogul and television personality known for his roles on American Idol and America’s Got Talent, has been candid about his experiences with cosmetic procedures, revealing a journey that spans Botox, filler, and unconventional wellness treatments.

In an April 2022 interview with The Sun, Cowell admitted he once went “a bit too far” with cosmetic enhancements, at one point failing to recognize his own reflection. “I saw a picture of me from ‘before’ the other day, and didn’t recognize it as me first of all,” he said, noting that many in Hollywood were “having their faces pumped full of this and the other” at the time.

Botox, Filler, and Facials

While Cowell has not publicly confirmed undergoing any major surgeries, he has openly discussed his use of Botox and filler. In a December 2025 interview with the Daily Mail, he reflected on the balance between appearance and authenticity:

“I’ve had Botox. I’ve had lasers and this and that. I look at myself sometimes and go, ‘Oh God, I look s—-.’ Sometimes I think I look okay. I’m more about, ‘Is it real?’ I guess that authenticity was my main thing.”

He also clarified that he has never had a facelift.

Controversial Blood-Cleansing Treatment

Beyond traditional cosmetic treatments, Cowell has experimented with a controversial blood-filtering procedure at a wellness clinic in Spain. In a December 2025 interview with British GQ, he described the three-and-a-half-hour treatment as akin to “going into Frankenstein’s laboratory.”

“It comes out of your body through these tubes… and apparently removes all the toxins,” he explained, calling the process “really painful” but worthwhile. Cowell emphasized his focus on internal health over surface appearance: “Botox is great, but it’s what’s on the inside that really matters.”

Similar blood-filtering treatments have recently gained attention among celebrities, including Orlando Bloom, though their effectiveness remains unproven by scientific studies.

A Brief Consideration of Cryonics

Cowell also explored the idea of cryonics, a futuristic practice aimed at preserving the body at ultra-low temperatures for potential revival in the future. In November 2025, he told The New York Times:

“I did think for a while: We’re all going to eventually, unfortunately, die. What if then I’ll just freeze myself?”

However, fatherhood altered his perspective. After learning more about the procedure, he joked: “Then someone told me they chop your head off, and I think your brain’s frozen… it’s like: Forget it.”

A Health-Focused Shift

Fatherhood also influenced Cowell’s decision to stop getting facial fillers. He recounted a moment with his son, Eric, that made him realize he had gone “too far”:

“Eric was in hysterics… Enough was enough. There is no filler in my face at all now. Zero,” he told The Sun.

Today, Cowell prioritizes a healthy lifestyle, emphasizing exercise, hydration, and a nutrient-rich diet. He credits this shift, along with supplements and stress management, for helping him “age backwards.” His commitment to wellness intensified following a 2020 e-bike accident that left him with a broken back. Reflecting on the experience, Cowell told Today in May 2023:

“I was so unfit before the accident… Things like that, I believe happened for a reason.”
