Vraylar (cariprazine) is a strong antipsychotic medicine approved by the FDA to treat schizophrenia, bipolar disorder, and major depressive disorder (MDD). It works by balancing chemicals in the brain called serotonin and dopamine. This can improve mood, thinking, and behavior.

Although Vraylar is not officially approved to treat anxiety disorder, research shows it might help. Doctors sometimes prescribe Vraylar off-label to treat anxiety when other medicines have not worked well.

Approved Uses and Dosage of Vraylar

The FDA has approved Vraylar for adults to treat:

Schizophrenia, a long-term illness with symptoms like hallucinations and delusions.

Bipolar I disorder, including manic episodes (very high or mixed moods) and depressive episodes.

Major depressive disorder, as an add-on treatment if antidepressants alone are not enough.

Doctors usually start Vraylar at a low dose of 1.5 mg daily. This dose can be increased to a maximum of 6 mg daily based on the patient’s response.

Research on Vraylar’s Impact on Anxiety

Anxiety often occurs alongside depression and bipolar disorder. This makes treatment harder. Studies show that when Vraylar treats the main mood disorder, anxiety symptoms also improve.

In one 2023 study, 952 people with bipolar I depression took either 1.5 mg Vraylar or a placebo for 6 weeks. Those taking Vraylar had lower anxiety symptoms in both body and mind compared to those who took the placebo. Importantly, Vraylar did not increase anxiety in any group.

Another 2025 study looked at people with major depressive disorder who did not respond well to antidepressants. Adding Vraylar at either 1.5 mg or 3 mg reduced their anxiety symptoms significantly compared to placebo.

These findings show that Vraylar may help reduce anxiety when combined with other treatments or when treating bipolar and depressive disorders. More research is needed to know if it works for people with anxiety disorder alone.

Off-Label Use for Anxiety

Even though Vraylar is not approved for anxiety disorder, doctors may prescribe it off-label for anxiety symptoms. This is common when anxiety comes with schizophrenia, bipolar disorder, or depression.

Standard treatments for anxiety include talk therapy, antidepressants (like SSRIs), benzodiazepines, and sometimes beta-blockers or other antipsychotics. Vraylar may be used if these do not work well enough.

Because Vraylar can cause serious side effects and interact with other drugs, it is essential to consult a doctor before starting it. The doctor will review the patient’s health history and current medications to ensure safety.
