A widely prescribed and inexpensive medication used to treat gout, colchicine, is emerging as a potential tool in preventing heart attacks and strokes among people with cardiovascular disease. According to a new Cochrane review, the drug — traditionally used to manage inflammation caused by gout — may help reduce recurrent cardiovascular events without causing serious side effects.
Understanding the Link Between Inflammation and Heart Disease
Cardiovascular disease is not just driven by cholesterol or blood pressure — chronic low-grade inflammation plays a critical role in the progression of heart attacks and strokes. Colchicine’s anti-inflammatory properties position it as a promising candidate for reducing this ongoing risk. Researchers believe that targeting inflammation can improve cardiovascular outcomes, particularly in patients who have already experienced a heart attack or stroke.
Review Findings: Evidence from Over 20,000 Participants
The Cochrane review analyzed 12 randomized controlled trials involving nearly 23,000 participants with a history of heart disease, stroke, or previous heart attacks. Participants took low-dose colchicine (0.5 mg once or twice daily) for a minimum of six months. Most of the subjects were men (approximately 80%), aged between 57 and 74 years.
Half of the participants received colchicine in addition to their usual medical care, while the rest were given a placebo or no extra treatment. The findings were promising:

The drug did not increase the risk of serious adverse effects, though some patients experienced mild digestive discomfort, which tended to resolve on its own.

Despite the encouraging results, researchers caution that more work is needed. The current evidence does not clearly show whether colchicine reduces overall mortality, the need for surgical procedures like coronary revascularization, or improves quality of life. The review also noted gaps in data regarding hospitalization rates.
