New Galleri Test Could Augment Screening, Detect Cancers Earlier

A new multi-cancer early detection test shows potential for identifying dozens of cancers before symptoms appear. In this California-led study, researchers evaluated Galleri, a blood test developed by GRAIL, Inc., and its ability to detect more than 50 cancer types in asymptomatic adults.

Study Design and Population

The analysis involved 23,161 participants aged 50 years and older from the United States and Canada who reported no cancer symptoms.

Participants underwent standard screenings recommended by the U.S. Preventive Services Task Force, including breast, cervical, colorectal, and lung cancer screening, in addition to the Galleri blood test.

Key Findings

The Galleri test identified a cancer signal in 216 participants, with 133 confirmed cancer cases, yielding a positive predictive value of 61.6%.

The false-positive rate was reported as approximately 0.4%.

Among Galleri-detected cancers, 53.5% were Stage I or II, and 69.3% were Stage I to III.

About 75% of cancers detected in the trial group lacked standard US screening options.

The test demonstrated the ability to predict the organ of origin for detected cancers with 92% accuracy.

When Galleri was combined with standard screening, cancer detection increased by more than sevenfold, suggesting additive value.

Clinical and Practical Implications

The findings indicate that Galleri could fill unmet screening needs by identifying cancers at earlier, more treatable stages in individuals who may not have standard screening options.

Experts stress that Galleri should complement, not replace, existing screening programs.

The study emphasizes careful integration into clinical practice to avoid unnecessary costs or patient confusion.

Proponents argue that early detection can improve survival and reduce treatment burden, potentially guiding more efficient diagnostic workups when a cancer signal is detected.

Critics note that this analysis represents an initial, incomplete evaluation and does not include a head-to-head comparison with a non-tested control group under identical conditions. A larger, randomized analysis and longer follow-up are needed to determine the full clinical utility.

Limitations and Future Directions

The current analysis is interim, with follow-up limited to about one year. A more comprehensive assessment involving the full cohort of approximately 35,000 participants is anticipated.

Ongoing randomized studies, including NHS-Galleri trials, are expected to provide additional comparative data on Galleri’s performance relative to standard screenings.

Clinical Guidance for Practitioners

Multicancer early detection tests like Galleri may offer a supplementary tool to expand cancer detection, particularly for individuals who fall outside routine screening pathways.

Clinicians should interpret positive Galleri results within the context of comprehensive diagnostic workups and existing screening guidelines.

Patients should be counseled on the test’s intended role as an adjunct to, not a replacement for, standard cancer screenings.
