New Research Shows Male Brain Loses Tissue Faster Than Female

Recent evidence suggests that male brains shrink faster than female brains as people age, offering new insights into how sex may influence neurological decline. A large-scale analysis involving 4,726 cognitively healthy individuals revealed consistent yet modest differences between men and women in the rate at which brain tissue deteriorates over time.

Natural Brain Shrinkage and Its Connection to Alzheimer’s Disease

The human brain naturally loses volume with age. In individuals affected by Alzheimer’s disease, this shrinkage becomes significantly more pronounced, leading to severe reductions in brain volume. Interestingly, although women are diagnosed with Alzheimer’s at nearly twice the rate of men, researchers have found surprisingly little evidence linking this higher prevalence to faster brain shrinkage among women.

According to the study, female brains may actually retain gray and white matter for longer periods than male brains, suggesting that biological sex may play a role in how brain structures respond to the aging process.

Comparative Findings Between Men and Women

“If women’s brains declined more, that could have helped explain their higher Alzheimer’s prevalence,” said co-author Anne Ravndal, a neuroscientist at the University of Oslo in Norway, in an interview with Nature journalist Rachel Fieldhous. However, Ravndal’s team found the opposite to be true.

The research team analyzed over 12,000 brain scans from participants aged 17 to 95. Each individual had undergone at least two MRI scans, spaced roughly three years apart. After accounting for differences in brain size between sexes, results revealed that men experienced more widespread declines across several brain regions, including many areas of the cortex.

In contrast, women’s brains showed shrinkage in fewer areas, and their cortical thickness remained more stable as they aged. These findings point to a biological distinction in how male and female brains undergo structural changes during aging.

Age Differences and Brain Regions Affected

The study highlights significant differences between male and female participants in brain volume, cortical thickness, and surface area. Despite these findings, the researchers caution against drawing premature conclusions. More research is needed to fully understand how these structural differences affect cognitive performance and overall brain health.

In the study, no significant difference was found between men and women in the rate of volume loss within the hippocampus—a brain region critical for memory and learning, and one often linked to dementia. However, women began to show faster hippocampal decline in later life, once their longer life expectancy was considered. This pattern may reflect delayed aging rather than increased dementia risk.

Persistent Sex Bias in Brain Research

Despite decades of study, neuroscience still suffers from a significant sex bias. A 2019 analysis found that only about 5 percent of published research in neuroscience and psychiatry accounted for sex-based differences. This lack of representation has led to inconsistent findings across studies. Some research indicates faster declines in men, others in women, and many studies fail to control for hormonal or lifestyle factors that may influence brain aging.

The new research, led by the University of Oslo, aimed to provide clarity. It identified measurable sex-based differences across a wide array of neurological indicators, including total brain volume, subcortical volume, and cortical morphology. These findings underscore the importance of considering sex as a biological variable in brain research and clinical diagnostics.

Implications for Understanding Cognitive Aging

Scientists are only beginning to grasp how changes in brain shape and size influence cognitive decline. Some forms of shrinkage might not necessarily signal disease; in certain contexts, they could reflect adaptive restructuring of the brain. Nonetheless, understanding where and how the brain loses volume is crucial for interpreting the risk of conditions such as Alzheimer’s or dementia.

As Ravndal and her colleagues emphasize, their results highlight genuine sex-based biological differences in aging but also stress the need for deeper, longitudinal research. Establishing how these differences translate into cognitive outcomes could lead to more effective, personalized approaches in preventing or managing neurodegenerative diseases.
