A bold new diagnostic approach promises to dramatically reduce waiting times for prostate cancer detection. Scientists at Heriot-Watt University, working in collaboration with clinical teams, have developed an ultrasound imaging test that can spot most prostate tumours in under 20 minutes. Early trials suggest it could rewrite current pathways dominated by PSA tests, MRI scans, and time-consuming biopsies.
What’s the new test, and how does it work?
The method relies on a super-resolution ultrasound technique enhanced by injecting a contrast dye containing microbubbles into a patient’s bloodstream. As these microbubbles travel to the prostate, a high-resolution ultrasound system tracks their flow.
Because cancerous tissue often alters microvascular patterns, the test can highlight lesions more precisely. In initial patient trials at Edinburgh’s Western General Hospital, the sensitivity reached 94%, and the test runs in under 20 minutes—far shorter than the hours sometimes needed for MRI workups.
Besides speed, the cost advantage is striking: ultrasound equipment and contrast usage are estimated to cost only about a tenth of what MRI scans demand. Researchers hope that by refining biopsy targeting and enabling earlier, accurate diagnoses, this test could reduce unnecessary MRI referrals and streamline care.
Dr George Papageorgiou, head of Less Grey Imaging, said that by integrating ultrasound more fully into the diagnostic pathway, the system could help “ensure early diagnosis” and reduce regional variation in care. Innovate UK has already awarded the project a £370,000 grant to support further development.
Why this matters in the context of PSA testing
The current entry point for suspected prostate cancer in many health systems is a prostate-specific antigen (PSA) blood test. Elevated PSA levels may prompt MRI scans and, in many cases, biopsies.
But PSA testing has long been criticized for its lack of specificity: high PSA doesn’t always mean cancer (it could reflect benign enlargement, infection, or other factors), and some cancers do not produce a high PSA at all. This ambiguity leads to overdiagnosis, overtreatment, or delayed detection of aggressive tumours.
Because of these shortcomings, many guidelines urge caution with widespread PSA screening. In the UK, the National Screening Committee has not recommended a national PSA screening program, citing risks of harm, overdiagnosis, and uncertain mortality benefit.
Thus, a faster imaging test that can more reliably detect clinically significant cancers could transform the role PSA plays—not necessarily replacing it, but supporting a more precise, less invasive cascade of testing.
How this aligns with other advances
Parallel developments in prostate imaging are already reshaping diagnostic strategy:
A recent trial (called PRIME) tested a shorter, two-part MRI scan lasting just 15–20 minutes and found it matched the diagnostic accuracy of a longer three-part scan, suggesting MRI pathways may soon be far more accessible.
The OPTIMUM trial evaluated micro-ultrasound guided biopsy (microUS) and found it comparable to MRI in identifying lesions that should be biopsied.
Researchers are also developing artificial intelligence systems that combine MRI and ultrasound imaging modalities to outperform human interpretation in tumour targeting.
In one multi-institutional AI model combining MRI and transrectal ultrasound, the hybrid system achieved higher specificity than radiologists while retaining sensitivity for clinically significant lesions.
These innovations, including the 20-minute ultrasound test, contribute to a broader shift: toward imaging-driven decision making, less reliance on blind or empirical biopsies, and more tailored screening.
While early results are promising, the new ultrasound test is still in initial clinical phases. Larger, multicentre studies will be needed to confirm performance, define false positive and negative rates, and compare outcomes with existing pathways. Its role relative to PSA (as a triage test, adjunct, or replacement) must be carefully worked out to avoid new forms of overdiagnosis or underdiagnosis.
Still, in a landscape long frustrated by delays, costs, and uncertainty surrounding PSA testing, a rapid, accurate imaging tool could be a game changer—streamlining prostate cancer detection, reducing burden on MRI services, and enabling earlier, better-informed treatment decisions.
