The Role of Online Therapy in Treating Stimulant Use Disorder

Stimulant addiction is emerging as a serious and growing public-health threat — one that is increasingly fatal but also not without hope for treatment. In recent years, the rise in use of cocaine, methamphetamine, and prescription stimulants has been accompanied by a disturbing rise in overdose deaths, often in combination with opioids. Primary care providers, armed with screening strategies and off-label pharmacologic approaches, may have a critical role in reversing the trend.

The Growing Stimulant Problem

In 2022, over 10.2 million people age 12 and older reported misusing some form of stimulant — whether cocaine, methamphetamine, or prescription stimulants — marking an increase of about a million from the prior year. Roughly 4.5 million now meet the diagnostic criteria for stimulant use disorder.

This surge is happening at a time when methamphetamine purity is peaking and prescriptions of stimulant medications (e.g. for ADHD) are climbing. While therapeutic use of ADHD medicines does not inherently lead to abuse, the expanded supply raises the risk of diversion (i.e. sharing or misuse) and misuse by others.

Meanwhile, stimulants are now implicated in 59 % of fatal overdoses (often in mixed use with opioids), and about 15.9 % of overdose deaths are attributed purely to stimulants. The so-called “fourth wave” of the overdose epidemic reflects the rising burden of stimulant-involved fatalities.

Compounding the danger, many stimulants are now adulterated with fentanyl, intensifying risk: overdose deaths involving both fentanyl and stimulants have increased more than 50-fold since 2010.

Why Stimulants Kill — and How Risks Accumulate

Unlike opioids, stimulants do not suppress breathing directly. Instead, they trigger cardiovascular stress: arrhythmias, heart attack, hypertension, and other acute risks. These become especially dangerous in older adults or in those with preexisting heart disease — populations commonly seen in primary care.

Stimulants can also precipitate psychiatric crises: high doses of prescription amphetamines have been linked to a more than fivefold increase in new-onset psychosis or mania. Thus, misuse does not just risk overdose but can provoke severe mental health complications.

Detection & Screening in Primary Care

Given the shifting overdose landscape, primary care should adopt systematic screening for substance use. A brief question such as, “How many times in the past year have you used an illegal drug or used a prescription medication for non-medical reasons?” can detect unhealthy use. Any nonzero answer (or hesitation) warrants follow-up.

If positive, clinicians can explore criteria consistent with DSM-5 definitions of substance use disorders: craving, unsuccessful efforts to cut down, interference with life roles, and loss of control over use.

Screening must be followed by a nonjudgmental, open conversation about use patterns, motivations, and readiness to change.

Treatment Options: Behavioral First, but Emerging Medication Tools

The cornerstone of stimulant addiction treatment remains psychosocial and behavioral therapies: cognitive-behavioral therapy (CBT), contingency management (i.e. reinforcements for abstinence), and supportive counseling. Because of limited specialist access, primary care providers may need to integrate or refer into these interventions.

Pharmacotherapy is more experimental: no medication is yet FDA-approved specifically for stimulant use disorder. Still, off-label strategies show promise:

Bupropion: a well-known antidepressant and smoking cessation aid, bupropion boosts dopamine and norepinephrine and may disrupt stimulant reward pathways. It seems most effective in users who don’t use every day.

Bupropion + naltrexone: in daily methamphetamine users, combination therapy with injectable naltrexone (an opioid antagonist) showed a modest but statistically significant reduction in methamphetamine-positive urine results.

Mirtazapine: has shown weak signals toward reducing methamphetamine use; side effects include weight gain and sedation — sometimes useful for reversing stimulant-associated weight loss.

Topiramate: this migraine and epilepsy medication may help in cocaine and alcohol use disorders, especially when combined with CBT. However, it has risks (kidney stones, cognitive side effects) and does not appear effective for methamphetamine addiction.

In addition to medications, mindfulness-oriented therapies and support groups (e.g. SMART Recovery) can strengthen recovery and resilience.

The Promise of Online Therapy and Telehealth in Addiction Care

The delivery of addiction treatment has shifted remarkably in recent years. Telehealth models — including online therapy — allow patients to engage with counseling, psychotherapy, and even parts of care remotely.
