AIIMS Launches India’s First Facial Transplant Program, Opening New Chapter in Reconstructive Plastic Surgery

India is set to take a significant step in the field of plastic surgery with the All-India Institute of Medical Sciences (AIIMS) in Delhi launching the country’s first facial transplant program. The initiative promises new hope to individuals living with severe facial deformities caused by acid attacks, burns, traumatic injuries, or road accidents. The facial transplant procedure involves replacing damaged facial tissue with skin, muscle, bone and nerves donated by a deceased person, potentially restoring essential functions such as speaking, eating and expressing emotion that cannot be achieved through traditional reconstructive surgery.

Dr Maneesh Singhal, head of the Department of Plastic, Reconstructive and Burns Surgery at AIIMS, said that while partial facial reconstruction has been performed previously, a full facial transplant will be a first for the institute. He noted that patients with major facial injuries often undergo many surgeries with limited improvement, facing both physical and social hardships. AIIMS plans to begin its first such surgery within a year, pending necessary regulatory approvals, and has established a multidisciplinary team to ensure success. This program also involves specialist training with experts from Brigham and Women’s Hospital, Harvard Medical School in the U.S.

Facial transplantation is a complex, multidisciplinary process that is much more than cosmetic surgery. According to Dr Indranil Sinha, an international expert in plastic surgery, the main goal is to restore vital functions such as eating, breathing, and smiling, not to simply alter appearance. The procedure is considered as a final option for patients who have lost important functions and for whom conventional methods are no longer effective. The process requires intensive planning, psychiatric screening to ensure mental readiness and long-term adherence to medication, as recipients must take lifelong immunosuppressive drugs to reduce the risk of tissue rejection.

Matching suitable donors is one of the biggest challenges for facial transplantation, as the recipient and donor must have similar skin tones and be of the same sex, while family consent and medical approvals must be secured quickly. Following surgery, patients require close follow-up, beginning with intensive care and ongoing monitoring for rejection or complications, which can arise in most cases but are usually manageable with proper care. Long-term results from centers abroad show that patients can live for many years after the operation, regaining much of their normal appearance and function.

Alongside these major developments in reconstructive surgery, advances continue in cosmetic techniques as well. The deep-plane facelift, an innovative method aimed at facial rejuvenation, is gaining popularity internationally. Experts like Dr Harrison Lee, a triple-board certified surgeon in the United States, highlight that techniques like these deliver more natural and lasting results compared to older methods, restoring youthful appearance while maintaining patient individuality. Dr Lee explains that newer methods focus on repositioning deeper facial tissues, offering subtle, long-term enhancements without an overly operated look.

As plastic surgery covers a wide range of medical procedures, from restoring lost function to enhancing appearance, the field is also responding to changing international guidelines and patient safety concerns. The American Society of Plastic Surgeons recently called for deferring gender-related surgeries on minors until adulthood, citing limited data on long-term effects and prioritizing patient well-being. This highlights the ongoing need for ethical standards, informed consent, and cautious decision-making as the specialty continues to evolve.

With India entering the field of facial transplantation and new advances in aesthetic surgery techniques becoming available, both patients and doctors are seeing an expansion of options for restoring both function and confidence. These programs signal continued growth in plastic surgery as a medical specialty serving a broad range of patient needs.
