Cosmo Pharmaceuticals has announced encouraging topline results from two large Phase 3 clinical trials of clascoterone 5% topical solution, raising hopes for a major advance in the treatment of male baldness. If approved, the medication could become the first hair-loss therapy in more than three decades to act through a novel biological mechanism, potentially benefiting over a billion men worldwide affected by androgenetic alopecia (AGA).
A New Approach to Male Hair Loss
Male AGA, the most common form of hair loss, is driven by the hormone dihydrotestosterone (DHT), which progressively shrinks hair follicles and shortens the growth cycle. Current treatment options—including oral 5-α reductase inhibitors and topical minoxidil—have remained largely unchanged for years. While effective for some, these therapies are limited by side effects, inconsistent results, and challenges with long-term adherence.
Clascoterone 5% topical solution offers a different approach. Rather than altering hormone levels systemically, it acts locally at the hair follicles, blocking androgen receptors and reducing DHT signaling where it matters most. By confining its effects to the scalp, the therapy aims to stimulate hair regrowth without the systemic hormonal risks associated with oral medications.
Robust Phase 3 Trial Results
The Phase 3 trials, SCALP 1 and SCALP 2, were conducted across multiple centers in the U.S. and Europe and enrolled 1,465 men over the age of 18 with AGA.
