In a recent interview with Urology Times®, Dr. Sia Daneshmand described the KEYNOTE-905 trial as “game-changing” for bladder cancer treatment. The study evaluated the combination of enfortumab vedotin and pembrolizumab, administered before and after surgery, compared with observation in patients with cisplatin-ineligible muscle-invasive bladder cancer.
KEYNOTE-905 is the first trial in this setting to show a survival benefit for a drug regimen, with favorable tolerability and no new safety concerns. Based on these results, the combination has received approval and is quickly becoming a new standard of care, Dr. Daneshmand noted.
Dr. Daneshmand, professor of urology at the Keck School of Medicine at USC and director of clinical research at USC Norris Comprehensive Cancer Center, also highlighted the growing collaboration between urologists and medical oncologists as perioperative systemic therapies expand.
“The two specialties are no longer separate,” he said. “We’re having a lot of multi-disciplinary discussions. Medical oncologists are becoming familiar with therapies for non–muscle invasive bladder cancer, while urologic oncologists are learning more about systemic therapies. These conversations help guide decisions both before and after surgery.”
He added that increased use of circulating tumor DNA (ctDNA) to guide treatment, broader knowledge of systemic therapies across specialties, and rising interest in bladder-preserving strategies are driving closer multidisciplinary coordination to optimize patient care.
