A new study from the Mayo Clinic, published in the New England Journal of Medicine, suggests that an off-the-shelf, dual-antibody therapy can deliver deep and durable responses in extramedullary multiple myeloma, one of the most aggressive and treatment-resistant forms of the disease.
“We are seeing powerful responses in a disease that historically has resisted every therapy,” said Shaji Kumar, M.D., hematologist at the Mayo Clinic Comprehensive Cancer Center and senior author of the study. “By recruiting T cells in two distinct ways at once, this dual-target antibody strategy can generate responses in patients who previously had very few effective options.”
The approach combines two engineered antibodies, talquetamab and teclistamab, which simultaneously engage T cells and trigger attacks on myeloma cells through separate immune pathways. Unlike CAR-T therapy, which requires custom manufacturing, this treatment is delivered via standard infusion-center injections.
In a trial of 90 patients, 79% responded to the therapy, and 54% achieved no detectable disease according to imaging or blood tests. Among responders, nearly two-thirds maintained disease control for at least one year—a notable improvement for a subtype of myeloma that typically carries only a months-long prognosis.
This study is the first large, prospective trial to focus exclusively on true extramedullary myeloma, using PET/MRI scans to define patient eligibility. Serious side effects were common, with infections being a significant concern, highlighting the need for comprehensive supportive care alongside immunotherapy.
Looking ahead, researchers aim to explore whether this dual-target strategy can be applied earlier in the disease course, improve safety through infection prevention, and expand the “two-locks, one-key” immune approach to other hard-to-treat cancers.
