Plant-Based Diet Prevents and Reverses Heart Disease in Hypertensive Rats, Study Finds

A study from the Neuroscience Institute at Georgia State University demonstrates that a plant-based diet rich in fruits, vegetables, nuts, and legumes can both prevent and reverse coronary microvascular dysfunction (CMD) in hypertensive rats.

Published in the Journal of the American Heart Association, this foundational research reveals that dietary intervention alone significantly improves microvascular health—a finding with particular relevance for women, who experience CMD more frequently and face worse outcomes than men.

Study Design and Dietary Intervention​

Researchers conducted a controlled experiment using spontaneously hypertensive rats, modeling human hypertension progression. Over six months, one group received a refined control diet while the experimental group consumed a plant-based diet comprising 28% fruits, vegetables, nuts, and legumes.

Both diets were nutritionally matched, with the key difference being the high antioxidant content in the plant-based regimen. A subgroup initially fed the control diet was later switched to the plant-based diet to assess CMD reversal.

The plant-based intervention produced significant benefits:

Prevention: Rats consuming plant-based diets from the outset did not develop CMD

Reversal: Hypertensive rats with established CMD showed restored microvascular function after dietary change

Clinical correlation: Coronary flow reserve measurements—a standard clinical metric—improved significantly

Tissue validation: Advanced cardiac MRI and vascular tissue analysis confirmed improved blood flow and reduced tissue damage markers

Mechanistic Insights and Gender Relevance​

“Since CMD disproportionately affects women and current treatments are only moderately effective, dietary approaches could offer a transformative strategy,” said lead investigator Dr. Rami S. Najjar, now at Emory University School of Medicine.

The high antioxidant content in plant-based foods likely counteracts oxidative stress—a key driver of microvascular damage in hypertension. This is particularly relevant for women, who face higher hospitalization rates post-diagnosis.

Translation to Human Nutrition​

The experimental diet translates to human equivalents including: 1 cup black beans, 1 large red bell pepper, 1.5 cups Brussels sprouts, 2 lemons, 1 medium sweet potato, 1.5 cups walnuts, and 1 cup blueberries daily. This combination provides synergistic phytochemicals and antioxidants that may explain the therapeutic effects observed.

Clinical Implications and Future Directions​

This study represents one of the first investigations into dietary management of CMD, a condition for which treatment options remain limited.

The findings support incorporating plant-based diets as adjunct therapy for microvascular dysfunction, particularly in hypertensive patients. Future research will focus on human trials and identifying specific bioactive compounds responsible for the observed cardioprotective effects.
