African-Caribbean men face a higher incidence and mortality risk, and a family history further elevates risk. Early detection is crucial in prostate cancer, which often develops without symptoms for years. Detecting cancer earlier through targeted screening could enable less invasive and more effective treatment, improving the chances of cure or long-term control. The model emphasizes inclusivity in men’s health, recognizing that African-Caribbean men and those in deprived areas are underrepresented in research and may face barriers to care. Overtreatment can cause significant side effects, including urinary incontinence and erectile dysfunction, as well as psychological distress. These harms can arise from both the cancer itself and subsequent interventions. Some cancers identified by PSA screening may never become life-threatening, raising concerns about net benefit in a population-level screening program.

From a health-system perspective, implementing a national PSA screening program would demand substantial resources—more testing, imaging, and specialist consultations—potentially diverting capacity from other needs. Estimates suggest the approach could increase testing volume and associated workloads by about 23%, posing a challenge to an already stretched NHS. Until a test can reliably distinguish life-threatening cancers from indolent ones, proponents caution that a nationwide screening program might cause more harm than good, reclassifying healthy individuals as patients and straining services without delivering clear, proportional benefits.

Ultimately, the debate centers on the balance between potential lives saved and harms from false positives, overdiagnosis, and overtreatment. A cautious path emphasizes better risk stratification, improved testing accuracy, and targeted outreach to underserved populations, while avoiding broad, population-wide screening until optimal tools and evidence are available.
