A recent genetic study published in Clinical, Cosmetic and Investigational Dermatology has investigated the causal relationships between popular diets—vegetarian, gluten-free, and low-calorie—and inflammatory skin diseases including psoriasis, acne, and psoriatic arthritis (PsA). The findings indicate that low-calorie diets may modestly increase the risk of developing psoriatic arthritis, while no significant relationships were found for vegetarian or gluten-free diets.
Background on Inflammatory Skin Diseases and Diet
Inflammatory skin diseases such as psoriasis, PsA, acne, and atopic dermatitis involve chronic immune dysregulation and inflammation, significantly impacting patients’ quality of life and mental health. Despite advances in treatments, recurrence remains common, prompting interest in lifestyle factors, particularly diet.
Popular dietary patterns have gained media and consumer attention due to perceived health benefits. Low-calorie diets are linked to metabolic improvements and longevity, vegetarian diets offer antioxidants and fiber, and gluten-free diets benefit individuals with gluten sensitivity. However, poorly balanced diets can cause nutritional deficiencies, potentially worsening skin health.
Limitations of Previous Studies and Mendelian Randomization Approach
Past clinical studies exploring diet and skin disease connections have yielded inconsistent results due to confounding variables and methodological limitations. This study used Mendelian randomization (MR), a method utilizing genetic variants as proxies for dietary exposures, which helps reduce bias and reverse causation, offering stronger evidence of potential causal links.
Study Design and Methods
Researchers analyzed genetic data from the UK Biobank (64,949 participants) and large genome-wide association studies involving up to 462,933 samples. They focused on three diet types (vegetarian, gluten-free, low-calorie) and four inflammatory skin diseases (acne, psoriasis, atopic dermatitis, and PsA).
Single-nucleotide polymorphisms (SNPs) significantly associated with each diet format served as instrumental variables. The study employed five MR methods, with inverse variance weighted (IVW) as the main analysis. Tests for pleiotropy, heterogeneity, and sensitivity analyses ensured result robustness.
Key Findings
The IVW Mendelian randomization analysis revealed that a low-calorie diet is associated with a slightly increased risk of developing psoriatic arthritis (odds ratio 1.05, 95% CI 1.01–1.10, p=0.008). Vegetarian and gluten-free diets showed no significant association with PsA or other inflammatory skin diseases, including psoriasis, acne, and atopic dermatitis.
Though some heterogeneity was observed in the vegetarian diet and psoriasis data, adjusted models confirmed no significant associations. Sensitivity tests did not indicate bias or pleiotropy.
Interpretation and Implications
The study suggests that prolonged calorie restriction could lead to nutrient deficiencies affecting immune regulation and inflammation, potentially elevating PsA risk. This challenges the belief that calorie restriction is always anti-inflammatory and highlights the complexity of dietary impacts on chronic inflammatory conditions.
Strengths and Limitations
The genetically informed MR design strengthens causal inference by minimizing confounding and reverse causation. However, limitations include low statistical power, lenient criteria for SNP inclusion, reliance on self-reported diet data, potential overlap between study samples, absence of multiple testing correction, and limited ethnic diversity, reducing the generalizability of findings.
Effect sizes were small, and the study lacked adjustments for multiple dietary factors, so results should be interpreted cautiously.
Understanding how calorie restriction influences immune balance may guide future prevention strategies for psoriatic arthritis and related inflammatory skin diseases.
