Though testosterone declines gradually starting in one’s 30s, symptoms tend to emerge in the 40s or 50s. Some younger adults with particular medical conditions can develop clinically low testosterone earlier.

How Is TRT Administered?

There is no one-size-fits-all approach. Common methods include:

Injectable testosterone: Delivers hormone directly, typically every few weeks.

Topical gels or creams: Absorbed through the skin.

Oral or sublingual formulations (less commonly used): Some newer formulations aim to preserve some natural testosterone production.

Hormone-stimulating medications: For men who want to preserve fertility, medications like clomiphene or hCG (human chorionic gonadotropin) may stimulate the testes to produce testosterone, often used in cycles.

Compounded products: Especially for women, customized creams or suspensions may be made by pharmacists, since off-the-shelf products are limited.

Treatment choice depends on goals (e.g. fertility vs symptom relief), sex, health status, and cost/insurance issues.

Potential Benefits of Testosterone Replacement Therapy

When used appropriately, TRT can lead to tangible improvements:

Enhanced libido, sexual performance, and vitality

Better mood, reduced irritability or depressive symptoms

Improved muscle strength, lean mass gain, and lower fat

Boosted bone mineral density, potentially lowering fracture risk

Increased red blood cell count (improves anemia in some cases)

Large surveys of users in the UK show that 85% of men reported TRT as “effective” or “very effective,” with 75% noting improved quality of life, 71% better mental wellbeing, and 69% enhanced confidence.

Clinical trials in older men have confirmed positive effects on sexual function, bone health, and anemia.

Though the use in women is less well studied, combining low-dose testosterone with exercise in post-fracture recovery did not significantly improve long-distance mobility, though some short-distance gains were seen.

Risks and Caveats You Should Know

As with any hormone therapy, there are potential downsides that require monitoring:

Cardiovascular concerns: Some early studies raised flags, but more recent data—including the TRAVERSE trial—suggest no increased risk of heart attack or stroke when carefully monitored. Still, the FDA now requires all testosterone product labels to include warnings about elevated blood pressure and related risks.

Blood thickening (polycythemia): If hemoglobin rises too high, it can increase clot risks.

Hormonal conversion and estrogen: In men, excess testosterone can be converted into estrogen, potentially causing gynecomastia or fluid retention if unchecked.

Fertility suppression: Exogenous testosterone can reduce sperm production, so it’s not ideal for men wanting to have children.
