Causes range from low sperm count and hormonal imbalances to abnormalities in sperm shape and movement. Yet while diagnostic clarity has improved, the emotional aftermath remains underexplored.

Estimates suggest around 48.5 million couples globally are affected by male infertility. Still, data collection in resource-poor settings is limited, and many health systems fail to include infertility services in universal coverage, leaving a critical gap in care.

Emotional Distress and Masculine Identity

The review underscores the psychological distress that often accompanies a male infertility diagnosis. Men report heightened symptoms of depression and anxiety, with depression closely linked to lower semen quality.

Many also experience grief, hopelessness, and deteriorating mental well-being over time—particularly when fertility treatments are prolonged or unsuccessful.

These challenges are compounded by societal beliefs equating fertility with masculinity. For many men, infertility is not merely a medical issue but a direct threat to their identity. Shame, self-doubt, and emotional withdrawal are common responses. This emotional burden often leads to reluctance in seeking help, creating a cycle of unaddressed psychological pain.

Impact on Sexual and Relationship Health

Beyond mental health, male infertility has significant effects on sexual function and relationship dynamics. Studies show a strong correlation between infertility and reduced libido, erectile dysfunction, and performance anxiety. These symptoms can lead to strained relationships and decreased intimacy.

Furthermore, the emotional fallout is magnified during assisted reproductive technologies (ART) procedures like intracytoplasmic sperm injection (ICSI), where men often feel sidelined in clinical settings.

Sociocultural Pressures and Lack of Support

In many cultures, fatherhood remains a cornerstone of male identity. Where male infertility is stigmatized or misattributed to external causes, affected men may experience heightened societal pressure and alienation. These sociocultural contexts can exacerbate mental health challenges, especially among those with lower income, limited education, or traditional gender beliefs.

Despite these challenges, fertility care often prioritizes women, leaving men with minimal psychological support. Emotional counseling tends to overlook male partners, even though they report comparable levels of distress. This disparity risks marginalizing men and may hinder both personal well-being and treatment outcomes.

Future frameworks must embrace a holistic, gender-sensitive model that addresses both the physical and psychological dimensions of infertility.

By doing so, healthcare providers can better support men navigating this often invisible struggle and contribute to improved reproductive and emotional outcomes for couples worldwide.
