Researchers from Mass General Brigham have identified a precise brain region that, when targeted during treatment, significantly improves outcomes for patients with essential tremor—a common neurological disorder affecting movement.
The findings, published in Science Advances, may enhance the safety and effectiveness of a cutting-edge, noninvasive procedure called magnetic resonance-guided focused ultrasound (MRgFUS).
Essential tremor affects roughly 1% of the global population and 5% of those over 60, causing involuntary shaking that can make everyday tasks difficult.
MRgFUS creates a small, permanent lesion in the thalamus, interrupting the brain circuitry that causes tremors. By analyzing data from 351 patients treated across three international hospitals, the researchers mapped an optimal subregion—referred to as a “sweet spot”—within the thalamus for maximum benefit and minimal side effects.
“The more precisely this target was treated, the better the patient’s outcome one year later,” said Dr. Melissa Chua, lead author and senior neurosurgery resident at Brigham and Women’s Hospital.
The model successfully predicted outcomes in an independent patient group, suggesting its reliability for broader clinical use.
Co-senior author Dr. G. Rees Cosgrove emphasized the life-changing impact of MRgFUS: “It’s like a gift when patients regain the ability to write, speak clearly, or drink from a cup.”
The team now plans further analysis to refine the procedure, aiming for even more personalized and durable tremor control.
This research represents the largest dataset to date for this treatment, marking a major step toward improved quality of life for those living with essential tremor.
